Introducción: En el sistema de salud colombiano se presentan barreras de acceso para la atención en salud bucal, como un mecanismo alternativo surge la Fundación Vive con Bienestar, que a través de la financiación con el Fondo Suizo Venture Philanthropy, implementó un programa en salud bucal “Sonrisas Saludables” en tres fases que incluyó además una investigación acerca de la experiencia para identificar lecciones aprendidas y recomendaciones.Objetivo: Describir la experiencia del Programa “Sonrisas Saludables” en Manizales, Colombia.Métodos: Investigación con enfoque mixto. La población fue de 1252 niños, sus padres o cuidadores y 32 actores vinculados al programa. Se realizó la revisión documental a las 1252 historias clínicas de los niños y se aplicaron encuestas de conocimientos, actitudes y prácticas a 240 niños y 173 padres, encuestas de satisfacción a 176 padres, entrevistas en profundidad a 32 padres y observación participante durante la atención clínica de los niños.Resultados: En la Fase 1 las historias clínicas permitieron reconocer que los niños están afectados por caries dental, maloclusiones y presencia de placa bacteriana, lo que los ubica en mediano y alto riesgo de caries. En la Fase 2 se realizó a los niños tratamiento acorde a diagnósticos y factores de riesgo. En la Fase 3 se atendieron necesidades de ortodoncia y ortopedia. En las entrevistas se identificó que padres y niños estaban satisfechos con el programa, lo más valorado fue la atención de ortopedia por ser un procedimiento que no está incluido en el plan de salud en el país.Conclusiones: Las intervenciones educativas grupales, estuvieron centradas en un modelo participativo a través de la estrategia Información, Educación y Comunicación, durante todas las fases del programa. El tratamiento especializado de ortopedia y ortodoncia, fue un factor diferenciador para el ingreso y la satisfacción.

Introduction: Scientific studies about the physical and mechanical properties of composite resins are increasingly needed in view of the wide variety of products available in the market today, resulting from the increase in their demand for dental care purposes, with a preference for minimally invasive preparations aimed at restoring the esthetic and functional characteristics of the treated element.Objective: Evaluate and compare the microhardness and porosity of the photopolymerizable composite resins Filtek Z350 XT and Zirconfill Technew.Methods: Ten test bodies were developed per sample (n= 10) and subjected to Vickers microhardness and porosity testing by computerized microtomography.Results: With respect to the presence of porosity, expressed in percentages, the Filtek group had the lowest mean values. When compared statistically, the two groups showed a statistical difference (p= 0.019) with a high effect magnitude. Regarding microhardness, the Filtek group had the highest mean values. Statistical comparison did not find any difference between the groups (p> 0.05).Conclusions: Porosity was significantly different in the groups studied, the Filtek Z350XT group exhibiting the lowest percentage. However, the groups did not show any statistical difference concerning microhardness, despite the fact that the Filtek Z350XT group had a higher mean value.

Introduction: The usage of prognostic tools that allow assessing trauma severity, as well as injury survival and mortality, allows patient stratification and making right decisions for the most effective treatment in the hospital area.Objective: To carry out a bibliographic review about the prognostic tools most widely used in emergency care to identify maxillofacial trauma severity.Methods: A bibliographic review was carried out from October 2018 to January 2019. We examined books and articles published in high-impact magazines, in Spanish, English and Portuguese, with priority attention on those five years old or less. We consulted the MEDLINE, EBSCO, PubMed, SciELO, Clinical Key databases using the descriptors Trauma Severity Indices, Maxillofacial Injuries, Traumatology, and Indexes. Two books and 50 articles were retrieved, although the review was limited to only 33 which described best the elements studied.Information analysis and integration: The Cooter-David scoring system, the maxillofacial injury severity score, the facial injury severity scale, the facial fracture severity scale, and the ZS model are the main instruments used for existing severity forecast in the facial region.Conclusions: There literature includes scales, indexes and scoring systems for predicting the maxillofacial trauma severity in the emergency department, each one showing advantages that make them an adjunct tool for trauma care; however, they also have disadvantages that complicate their application in the hospital setting. The facial injury severity scale is among the most widely used tools, as far as it is easy to apply in the emergency services, but with deficiencies for treating soft tissue and bone components. In Cuba, there is little evidence about the use of these instruments in the specialty of maxillofacial surgery.

Introduction: The presence of atypical dentofacial structures may be the first indicator of other congenital defects related to syndromes of likewise genetic origin. Nance-Horan syndrome is a monogenic X linked disorder phenotypically characterized by distinctive dysmorphic dental and craniofacial alterations, congenital cataracts and cognitive deficit.Objective: Describe an unusual case of dental anomalies in the course of Nance-Horan syndrome.Case presentation: A case is presented of a white male 13-year-old patient. Clinical examination revealed a dysmorphic pattern characterized by long narrow facies, prominent ears with auricular anteversion, a big nose with a high nasal bridge, generalized diastema in both arches, and all the teeth microdontic and abnormally shaped. Periapical radiological examination found short roots and broad pulp chambers. The personal pathological antecedents of the patient, the dysmorphic craniofacial pattern and the radiographs correspond to characteristics of other cases of Nance-Horan syndrome reported in the literature. Mutation of the Nance-Horan syndrome gene is completely expressed only in males. Since males are hemizygous for X linked genes, one copy of the mutated allele is sufficient for the appearance of a sex-linked recessive inheritance disease.Conclusions: Evidence was found of the crucial importance of conducting careful examination, both clinical and radiographic, of patients with dental anomalies. Emphasis is placed on the joint work of various disciplines and specialties, both medical and dental.

Introduction: The outbreak of coronavirus disease 2019 (COVID-19) has been declared a worldwide public health emergency. Dentists are the health care providers most directly exposed to this condition, since their area of activity is the mouth, a location recognized as the site through which the disease is transmitted.Objective: Clarify some notions about COVID-19 and the measures to be taken to prevent transmission to dental care providers.Main remarks: A single method has not been discovered which meets all the requirements concerning infections associated to medical care. In the struggle against COVID-19 an important role is played by standard precautions, i.e. measures applied to all patients, irrespective of their diagnosis or whether they are known to have an infection or be colonized by a given agent. SARS-CoV-2 is an enveloped virus, a characteristic that makes it more sensitive to the action of disinfectants.General considerations: COVID-19 presents non-specific symptoms, the most common of which are fever, anorexia and dry coughing. Transmission probably occurs by the spread of the virus in the air and by direct contact. Implementation of standard precautions, among which are hand hygiene, the use of personal protection equipment and care for the environment, will lead to a reduction in the chances of contagion and will contribute to control the spread of the disease. It is important for medical care providers, among them dental care professionals, and the public at large to know about this new virus, so that coordinated, timely and effective actions may be taken which will help prevent the occurrence of new cases or bad results in the struggle against this pandemic.

Introducción: Las anomalías dentofaciales son trastornos del crecimiento dental y facial que afectan tanto a niños como a adultos. Objetivo: Presentar un caso clínico, en que en la búsqueda de armonía, por una anomalía dentofacial, se aplicaron principios del manejo multidisciplinario de cirugía ortognática y de otoplastia.Caso clínico: Paciente masculino de 18 años de edad que refiere inconformidad estética y en el que, mediante el análisis de modelos de estudio, mediciones de las telerradiografías indicadas y los hallazgos del examen físico, se llegó al diagnóstico de prognatismo mandibular con exceso vertical del mentón. Se ejecuta tratamiento ortodóncico-quirúrgico, realizándose osteotomía sagital mandibular y mentonoplastia de reducción de altura. Se emplearon como medios de fijación interna los tornillos bicorticales en la zona de ángulo mandibular y miniplacas en la región del mentón. Tres meses después se realizó otoplastia, para corregir la presencia de orejas prominentes, con el logro de un resultado funcional y estético satisfactorio. Conclusiones: Con la aplicación de los principios del manejo multidisciplinario de cirugía ortognática, combinada con la realización de una otoplastia, los resultados fueron positivos. Se alcanzó el objetivo de brindar armonía facial, con mejoría funcional y estética, y una alta satisfacción del paciente.

Introduction: Gastroesophageal reflux disease is a functional multifactorial disorder of the upper gastrointestinal tract. Oral alterations are considered to be extraesophageal manifestations of this disease.Objective: Determine the oral alterations found in patients with gastroesophageal reflux disease.Methods: An observational descriptive study was conducted of 37 patients diagnosed with gastroesophageal reflux disease. The variables analyzed were age, sex, presence of alteration, signs and symptoms, and location. A database was developed with the information recorded, and absolute frequencies and percentages were estimated.Results: Of the patients studied, 48.6% had some sort of oral alteration, such as a burning sensation (40.5%) and erythema (35.1%). The regions affected included the soft palate / uvula (72.2%) and the tongue (61.1%).Conclusions: Almost half of the patients affected by gastroesophageal reflux had some sort of oral manifestation, the most common of which were a burning sensation, erythema, a dry mouth sensation and ulcerations. The most common locations were the soft palate / uvula and the tongue.
