Objective: Describe the clinical management of an atypical Le Fort I fracture case.Case presentation: A 33-year-old patient was brought to the Surgical and Maxillofacial Traumatology Service of Humberto Lucena Senatorial Emergency and Trauma Hospital in Joao Pessoa (PB), Brazil, after undergoing physical aggression. The patient was conscious and lucid. Clinical examination revealed sinking of the middle third of the face with edema and bilateral periorbital ecchymosis. Visual acuity and ocular motricity were preserved in both eyes. A cut-contusion injury on the upper lip, maxillary mobility when handled and discrete occlusal dystopia were also observed. Imaging examination identified a high bilateral Le Fort I fracture. The therapeutic approach chosen was fracture reduction and fixation with plates and screws. After a period of two months, the patient is healing well without any apparent functional or aesthetic alteration.Conclusions: Atypical Le Fort I fractures are not frequent; however, a number of etiologies besides the impact force may bring about them. Treatment should be based on the one for classical Le Fort I fractures with adjustments to the surgical access approach.

Introduction: The oral health of young adults is a basic human right, and its contribution is fundamental for the enjoyment of a good quality of life.Objective: Determine oral health at a Peruvian police academy from July to August 2017.Methods: An observational descriptive cross-sectional study was conducted. The sample was 276 young adults randomly selected according to inclusion and exclusion criteria and in compliance with the ethical standards of scientific research. Oral health was evaluated with the aid of an epidemiological card containing the following indicators: CPOD index, dental caries significance (DCS) index, simplified oral hygiene index (OHI-S), and WHO dental prosthesis need and situation. The evaluation was conducted by a qualified observer under natural light. Data were processed with the software STATA v 14, using frequency distribution tables and figures.Results: Dental caries prevalence was 63.76% (CPOD= 3.86; DCS= 9.64), OHI-S: 2.02 (SD= 0.41). Fixed partial dentures were the most commonly used, and they were required in both jaws.Conclusions: Dental caries experience was moderate, oral hygiene status was fair, and fixed partial dentures were the most commonly required for both jaws. Although the indicators are heartening, there is still an urgent need to implement oral health policies in the police community.

Introduction: The placement of dental implants is based on the creation of optimal conditions in the remnant bone. In some cases, it is indispensable to perform bone regeneration procedures and use barrier membranes to create such optimal conditions capable of favorably supporting the dental implant.Objective: Describe alternatives of barrier membranes in cases of guided bone regeneration.Case report: A female 53-year-old patient presents with a gingival fistula attached to tooth 14. Root fracture is diagnosed and extraction is conducted. Next, alveolar biomodification is performed to carry out guided bone regeneration and placement of a fibrin-rich plasma membrane. When healing is complete dental implants will be placed.Conclusions: The use of barrier membrane alternatives has shown to be effective in cases of guided bone regeneration.

Introduction: Sialolithiasis is a common disorder of the major salivary glands which presents more frequently in male patients.Objective: Present a clinical case of sialadenitis due to sialolithiasis of the submandibular gland successfully treated by intraoral canicular sialolithectomy.Case report: A male 45-year-old patient attending the Maxillofacial Surgery Department at Manuel de Jesús Cedeño Specialty Clinic in Granma, Cuba, presents with discomfort on the floor of the mouth and a hard, mobile and easily movable mass of increased volume. Radiographic examination revealed a radiopaque oval-shaped image of well-defined edges, approximately 4 cm long x 2 cm wide. A diagnosis was made of sialadenitis due to sialolithiasis of the right submandibular gland. Symptoms were treated with analgesics / anti-inflammatories and antibiotics. Intraoral canicular sialolithectomy was performed. Rechanneling of the duct was then conducted to allow permeabilization.Conclusions: Sialadenitis due to sialolithiasis is a condition of a favorable prognosis. Appropriate treatment will have the gland return to its normal physiology, improving the patient's quality of life. In our case, surgical removal was conducted by intraoral canicular sialolithectomy without any complications. The patient had completely recovered after one month's evolution, the treatment being satisfactory both short- and long-term.

Introduction: Facial injuries may affect patients' capacity to eat, speak and interact.Objective: Characterize severe maxillofacial trauma in the emergency departments of General Calixto García University Hospital and Miguel Enríquez Hospital in Havana, Cuba.Methods: A descriptive cross-sectional multicenter study was conducted in the emergency departments of General Calixto García University Hospital and Miguel Enríquez Hospital from March 2017 to March 2019. The study universe was patients diagnosed with severe maxillofacial trauma. The qualitative variables were sex, etiology, consumption of alcoholic beverages and injuries, whereas the quantitative variable was age. A data collection form was developed and the information was stored in a database. Data were expressed in whole numbers, percentages and statistical tables. A search was performed for the relationship between the variables.Results: Male sex constituted 80.6% of the sample. The best represented age group was 18-30 years (33.3%), followed by 31-40 (25.0%). In 41.7% of the patients the trauma was related to traffic accidents, and in 30.6% to interpersonal violence. Alcohol breath was detected in 51.9% of the patients, among them most of those cared for at Miguel Enríquez Hospital. There was a predominance of wounds larger than 2 cm, followed by wounds under that length (61.1% and 32.4%, respectively) and mandibular fracture (51.9%).Conclusions: A predominance was found of the male sex and the 18-30 years age group; the main etiology was traffic accidents, followed by interpersonal violence; the most common soft tissue injuries were wounds larger than 2 cm, whereas the most common hard tissue injuries were mandibular fractures. A significant association was found with consumption of alcoholic beverages among the patients treated at Miguel Enríquez Hospital.

Introduction: Malignancy of dentigerous cyst into primary intraosseous carcinoma is infrequent and scantily documented in the literature. It represents 1% to 2.5% of the total odontogenic tumors and is exclusive of maxillary bones. It is more common among men aged around 50 years. Its clinical characteristics are edema, tooth mobility and paresthesia, and in most cases an absence of pain. These features hamper its diagnosis and pose a challenge to pathologists.Objective: Review the prevalence of primary intraosseous carcinoma derived from dentigerous cysts as published in PubMed in the last 15 years.Methods: A literature review was conducted of papers published in the last 15 years. The database PubMed was consulted using the following search terms: “dentigerous cyst prevalence”, “primary intraosseous squamous cell carcinoma”, “dentigerous primary intraosseous squamous cell carcinoma”. The papers included were in English or Spanish. Of the 217 papers obtained, 39 were selected for data crossing.Data analysis and integration: Recognition of a condition as aggressive as primary intraosseous carcinoma derived from a dentigerous cyst requires examination of its clinical and radiographic characteristics as well as its symptoms. Its close relationship to dentigerous cyst, the most prevalent of developmental cysts, makes it necessary to gain an accurate and deep understanding of both. Primary intraosseous carcinoma poses a challenge to clinicians due to its low symptomatology and poor survival. Of the total 44 823 odontogenic cysts studied, 9 806 were diagnosed as dentigerous cysts and 22 maligned into primary intraosseous carcinoma, for 0.32% of the total.Conclusions: Malignation of a dentigerous cyst into primary intraosseous carcinoma has a low incidence and a silent presentation, which hampers its early diagnosis. Protocol biopsy is recommended for each case of dentigerous cyst to prevent the poor survival caused by primary intraosseous carcinoma.

Introducción: El té verde (Camellia sinensis) y el propóleo presentan flavonoides, que inhiben el crecimiento, metabolismo y la coagregación del Streptococcus mutans, principal agente causal de la caries dental. Objetivo: Evaluar la actividad antibacteriana in vitro del extracto etanólico del té verde (Camellia sinensis) al 10 % y 20 % comparado con extracto etanólico de propóleo al 10 % y 20 %, frente al crecimiento de cepas de Streptococcus mutans (ATCC 25175).Métodos: Estudio experimental in vitro, longitudinal, prospectivo y comparativo. El universo estuvo constituido por 90 discos de difusión y la muestra por 15 discos embebidos en té verde (Camellia sinensis) o propóleo a diferentes concentraciones, clorhexidina acuosa al 0,12 % y agua destilada. El tamaño de muestra se calculó por fórmula de comparación de medias, después de realizar un estudio piloto. Se colocaron los discos de difusión embebidos en las sustancias sobre agar Mueller Hinton, sembrado con Streptococcus mutans (ATCC 25175), haciendo medición del ancho de los halos inhibitorios a las 24 y 48 h. Se aplicaron pruebas de comparación no paramétricas de Kruskal Wallis y la prueba rangos de Wilcoxon. Resultados: El máximo ancho de halo inhibitorio logrado por clorhexidina acuosa al 0,12 %, extracto etanólico de té verde (Camellia sinensis) al 20 % y extracto etanólico de propóleo al 20 % fue a las 24 h con valores de 10,64 mm ± 0,924 mm, 6,82 mm ± 0,982 mm y 8,36 mm ± 1,286 mm, respectivamente. El extracto etanólico de té verde (Camellia sinensis) al 20 %, presentó diferencias estadísticamente significativas respecto al extracto etanólico de propóleo al 20 %, tanto a las 24 h (p= 0,013), como a las 48 h (p= 0,011). Conclusiones: Frente al crecimiento de cepas de Streptococcus mutans (ATCC 25175), el extracto etanólico de propóleo al 20 % presenta mayor actividad antibacteriana respecto al extracto etanólico de té verde (Camellia sinensis) al 10 % y 20 %, actividad que disminuye con el paso del tiempo.

Introduction: The oral health of students from rural areas is a priority public health problem in Peru.Objective: To determine socioeconomic factors in relation to oral health in students from a rural Peruvian area.Methods: Descriptive cross-sectional observational study. The sample consisted of 604 students from a rural Peruvian area selected for convenience, meeting criteria of inclusion and exclusion, following ethical standards in scientific research. Socioeconomic factors were assessed using a structured questionnaire and the oral health using an epidemiological record with the indicators: DMFT index, Significant Caries Index (SiC), Simplified Oral Hygiene Index (OHI-S), Index of clinical consequences of untreated dental caries (PUFA). The evaluation was performed with natural light by calibrated observers. The data was analyzed in the program STATA 14 through frequency distribution tables and graphics. The relationship of each of the socioeconomic factors with the experience of dental caries was evaluated by the simple binary logistic regression tests.Results: The prevalence of dental caries was 85, 26 % (DMFT = 6, 60 SiC=7, 23). The most frequent oral hygiene condition was regular, the prevalence of untreated tooth decay was 61.75 % .Conclusions: The level of illiterate instruction, family responsibility and independent parental occupation showed a statistically significant association with their children's dental caries experience in a rural Peruvian area. (p < 0.001).
