Carpal Tunnel Syndrome is a public health problem that affects 10% of the world population with a high prevalence and incidence, which generates an increase in disability in working people, which results in absenteeism from work, and high costs. The aim is to evaluate the effectiveness of treatment with neurodynamics technics and oral corticosteroids in patients with carpal tunnel syndrome who attend the Physical Medicine and Rehabilitation service of the IAHULA in the period march-june 2022. Methodology a quasi-experimental transverse field study was carried out. Results: in this study 83.3% of the patients are female, 46.66% are in an age range between 50 and 59 years, with respect to occupation 43,3% were workers. According to ECN findings 80% has bilateral affection. In the VAS scale, when comparing the admission value and at 30 days, a p= 0.000 for group A and p= 0.002 for group B, significant p. In the DASH Scale, a significant p was found compared to the beginning and at 30 days for group A p=0.005 and for group B p=0.000. Conclusion: Neurodynamics technics decreased pain and increased functionality significantly with respect to the initial evaluation, at 15 days and a month of treatment in both groups, without differences in these.

Introduction: Hyaluronic acid has become the preferred and best accepted treatment by patients to correct imperfections on the face, or to harmonize it. There are multiple adverse effects described in its application as filler treatment. With ultrasound as a diagnostic test, it is possible to characterize the type of substance used. In our case, an investigation was carried out where the main objective was to identify the ultrasound changes with the use of hyaluronidase in a patient with late complications after the application of hyaluronic acid as filler material. Materials and methods: The research design is observational, prospective, it was carried out based on a clinical case. Discussion: Although it is true that hyaluronic acid is well tolerated by a large number of patients, the ideal filler material has not yet been determined. With the use of ultrasound, the signs suggestive of the filler material administered and its evolution over time are characterized. Complications range from pain, deformity to necrosis, which is why hyaluronidase has gained a leading role in the treatment of these complications. Conclusion: Ultrasound as a diagnostic method in the characterization of filler substances constitutes an essential tool to characterize the applied substance, its possible complications, as well as the use of hyaluronidase in the correction of these unwanted effects due to the use of hyaluronic acid, appreciating the changes at the level of the treated tissues.

Objective: To analyze vitamin B12 and its relationship with hematological disorders in late adulthood, evaluating biomarkers of greater interest and reliability in the diagnosis of vitamin B12 deficiency. Methodology: An analytical, cross-sectional, field design was carried out that relates the causes to each other, obtaining the indicators of vitamin B12 and hematological disorders in the elderly. We included 85 patients from the Tisaleo canton of the province of Tungurahua characterized by being adults aged 65 to 90 years, both men and women, regardless of ethnicity, race, or social-economic situation Results: No association was found between low levels of vitamin B12 with any hematological disorder, since the mean values of the sample are within normal parameters. However, decreased vitamin B12 concentrations were found at the 25th and 50th percentiles, but hemoglobin (HBG), hematocrit (HCT), and mean corpuscular volume (MCV) values were normal. At the 75th percentile, vitamin B12 levels were normal, but within the components of the blood count, mean corpuscular volume (MCV) was slightly elevated. Conclusions: Vitamin B12 is an essential substance that participates in different metabolic mechanisms of people. The study determined that levels of this vitamin are within normal parameters and was not associated with any hematological disorder.

Syringomyelia is a pathology characterized by the presence of a cyst inside the spinal cord. This pathology has an incidence of 8.4 new persons each year per 100,000 inhabitants. The average age of onset is approximately 30 years with an evolution period of 6 years. The following is the clinical case of a 64-year-old male patient with no clinical or surgical history of importance, who comes to the Emergency Department of Hospital General Ambato for paresthesia in lower limbs of progressive evolution, the physical examination shows an unstable gait with increased base of support, decreased strength, spastic paresthesia, inability to ambulation and standing, bilateral positive Babinski, bilateral patellar and bilateral Achilles hyperreflexia with sensitive level in T10. Simple magnetic resonance imaging of the dorsal spine was performed, showing an intramedullary cystic lesion extending from T2 to T9, reaching the diagnosis of Dorsal Syringomyelia not related to other anomalies such as Arnold Chiari Type I. The derivation of the syrinx to the subarachnoid space is a definitive surgical treatment, however, in some cases the procedure manages to reduce the size of the syringe but does not produce any noticeable improvement in the clinical condition of the patient.

Polyarteritis nodosa is a rare necrotizing vasculitis that affects medium and small caliber vessels, which can affect from one organ to systems of the human body, therefore, its clinical presentation is variable and its etiology is predominantly idiopathic; Regarding its diagnosis, other pathologies must be ruled out and the clinic, diagnostic and paraclinical images available must be correlated, in order to provide adequate and timely therapy, taking into account that pharmacological therapy helps to reduce damage and further progression but is not a cure. The case of a 46-year-old patient with rapidly progressive cutaneous polyarteritis nodosa that led to necrosis of the fingers of the 4 extremities is presented. It was possible to stabilize and stop its progression through recognition and pharmacological management with glucocorticoids and cytotoxic drugs, however, it required amputation.

Quality control in clinical laboratories is based on continuous improvement and reduction of errors, in order to guarantee the correct execution in the provision of diagnostic support services, for patient safety. The descriptive research had a quantitative approach to evaluate the technical and management requirements of the ISO 15189:2012 standard and its compliance in 80 clinical laboratories in the Ambato Canton through the analysis of data obtained through surveys carried out in each of the evaluating laboratories. The in-situ assessment was carried out in 84.7% of private laboratories in the Ambato Canton, with prior informed consent, with the application of an instrument (α=0.81) based on the Ecuadorian SAE accreditation criteria according to the ISO 15189:2012 standard. 38% of low-complexity laboratories and 33.3% of medium-complexity laboratories have a compliance percentage <75% of both management and technical indicators. 80% of highly complex laboratories comply with 100% of Management indicators and 79.4% of highly complex laboratories comply with 100% of Technical indicators, with a significant difference by type (p<0.05). It concludes by pointing out that quality assurance in a clinical laboratory should not be seen as an obstacle, but rather as an opportunity for improvement.

A les darreres dues dècades hem vist sorgir una important quantitat d'investigacions al voltant dels femicidis a Amèrica Llatina i el Carib. En aquest context, és important ressaltar el pioner lloc que ocupa Costa Rica en la genealogia del concepte, la seva elaboració teòrica, així com el passatge de tipus penal. El següent article presenta reflexions epistemològiques, polítiques i ètiques que es desprenen del meu procés de recerca doctoral sobre les tensions pel reconeixement i les lluites per la justícia al voltant dels femicidis al país. Metodològicament, l'anàlisi se centra en la feina d'entrevistes a profunditat que vaig fer amb familiars i amistats properes a dones víctimes d'aquesta violència extrema; així com el treball de monitorització, contrast i producció de dades estadístiques que va concloure amb l'elaboració d'una cartografia crítica de femicidis. A manera de conclusió, es convida a construir investigacions i polítiques vinculars, on es col·lectivitzin els efectes de llegir, escoltar, sentir i pensar el dolor social.
