Resumen
Introduction: Obstetric hemorrhage is one of the most common causes of maternal morbidity and mortality worldwide. Placenta previa is an important risk factor for obstetric hemorrhage. The purpose of this study is to determine the risk factors related to placenta previa in pregnant women Santo Domingo.
Methods: The cases were women whose deliveries were complicated with placenta previa and the controls were deliveries without placenta previa. 16 cases and 76 controls were analyzed. Statistical analysis was conducted and odds ratio (OR) was calculated.
Results: The relationship between placenta previa with a history of two or more abortions with an OR = 5.3 was demonstrated; with previous caesarean section OR = 3.02, which increased with more than 2 caesarean sections with an OR = 5.91, with previous ectopic pregnancy and diagnosis of placenta previa in a previous pregnancy. Patients with placenta previa and a history of surgery had a higher risk of requiring a hysterectomy. Women with placenta previa and a history of multiple caesarean sections (2 or more) had a higher risk of suffering from placenta accreta with an OR = 18.
Discussion: Our study suggests that multiple abortions, previous caesarean section, multiple previous caesarean sections, antecedents of placenta previa and history of ectopic pregnancy are risk factors of developing placenta previa.

Resumen
Introduction: Growth hormone deficiency (DGH) is the endocrine cause more important and frequent of short stature. This hormonal deficit is accompanied in addition to a delay in bone maturation.
Method: An applied, retrospective and descriptive study of 108 patients with the diagnosis of low pituitary height due to growth hormone deficiency, among whose purposes were to determine the behavior of ripening skeletal according to sex, diagnosis of hormonal deficit, age of start of treatment, and type of pituitary hormonal deficit.
Results: There is a marked delay in bone age in patients with DGH, which is greater if the deficit is congenital, total and selective.
Conclusions: It is highlighted that patient with DGH, regardless of the type of this, showed a marked delay in bone age, as well as some general considerations of the subject.

Resumen
Cancer is a global health problem. Multiple factors related to the disease and treatments cause a special predisposition to infections in the cancer patient. Sepsis is an important cause of morbidity and mortality. A descriptive cross-sectional study was carried out, where all sepsis events confirmed by microbiology were included, in patients with solid tumors in chemotherapy, treated at the Medical Oncology service of the Hospital Clinico Quirúrgico Hermanos Ameijeiras, from January 2015 to January 2017, whose objective was to describe their behavior. The mean age was 53 years (32.3%) and male patients predominated (56.5%). Digestive tract tumors, lung carcinomas, and soft tissue sarcomas accounted for 32.3, 25, and 13.7%, respectively. Of the 124 sepsis events, 39.5% had febrile neutropenia, followed by urinary sepsis (34.7%). In the group of patients with other infections, inflammatory novel lung disease predominated, followed by grade IV mucositis. There was no significant statistical association between the chemotherapy schedule and the appearance of infection. The advancement and improvement of the treatments have prolonged the life of the patients and this has favored the better study and understanding of the different adverse events related to the administration of chemotherapy.

Resumen
An observational, descriptive and cross-sectional study of prospective data collection was carried out, whose objective was to determine the correlation between total serum bilirubin and transcutaneous bilirubin in newborns admitted to the neonatology area of the Maternidad Nuestra Señora de la Altagracia University Hospital.
The study group consisted of a total of 30 neonates with a request for laboratory examination of serum bilirubins, who were jointly taken transcutaneous thoracic and frontal bilirubin with no more than 30 minutes of difference.
In this, it was obtained that the mean values of Bttf, Bttc and the gold standard were 15.56mg/dl, 14.53mg/dl and 12.89mg/dl respectively, and the Pearson correlations were the following: r=0.71 for the transcutaneous measures, r=0.91 for frontal measurement and serum bilirubin, and r=0.78 for Bttc and bilirubin in blood. The discrepancy between Bttf/serum was 2.67mg/dl and between Bttc/serum of 1.64mg/dl. Therefore, it was concluded that serum and transcutaneous bilirubin values have a good correlation and can be used as a diagnostic method.

Resumen
The objective of the study was to characterize the clinical profile of congenital hypothyroidism in patients under 13 years of Pediatric Endocrinology Consultation of the Children’s Hospital Doctor Robert Reid Cabral. A descriptive, observational, cross-sectional study was performed with prospective collection of data, where 22 patients were evaluated and their clinical records were reviewed, during the period July-December 2017. At the conclusion of the investigation it was determined that 50 % of the Patients were diagnosed with congenital hypothyroidism before 3 months of age and 18 % above 30 months. Macroglossia and umbilical hernia were the main clinical manifestations at the time of diagnosis (40.9 %). The TSH measurement was used in 100 % of the cases. Ultrasonography was the most used image study. In 68.1 % of patients with HC, the cause of HC has not yet been determined. The family history of thyroid disease and Down syndrome stood out among the risk factors present in the study population. 59 % of patients have had a permanent clinical evolution. The patients evaluated did not have auditory deficit. In the psychological evaluation we used the Development Profile test (DP3), carried out by the Department of Mental Health of the Dr. Robert Reid Cabral Children’s Hospital with direct interview with the parents, and 40.9 % of the patients obtained average level in the motor, adaptive, social-emotional and communicative scale of its development. The cognitive level was the executive function most affected, it was very low in 45.4 % of the patients. The overall rate of development was very low at 45.4 %. 75 % of patients diagnosed after 30 months had a very low overall developmental rate. 72.7 % of the patients diagnosed before the first 3 months of life had an average development index.

Resumen
Introduction: Congenital heart disease (CHD) is a group of pathologies where the structure or morphology of the heart is affected, that is, it occurs a disorder in the function of the organ, originated during the formation or embryonic cardiovascular development. CC represent the most common congenital malformation in children worldwide, with a prevalence of 4-14 per 1000 live births, with an increase in Asian countries in 13-16 / 1000 live births.1-4,6
Material and Methods: A descriptive and cross-sectional study was carried out, with retrospective data collection from secondary sources, using the cardiology consultation database of the Dr. Hugo Mendoza Pediatric Hospital between January 2017 and August 2019.
Results: The age range of the patients: less than 1 month 40.7%; 1 month to 1 year 18%; 1-4 years 23.4%; 5-9 years 7.6%; 10-14 years 7.6%; over 15 years 2.8%. Heart disease affected both sexes interchangeably. According to the type of heart disease, the Cornflower with 92.1%, and the Cyanogen with 7.9%.
Conclusions: During the study period, there were significant differences between the number of cases received annually in this center, of 8/1000, 1/1000, 6/1000 patients treated for 2017, 2018, 2019 respectively, reducing that there is no high annual prevalence for patients with this type of diseases in this center.

Resumen
Introduction: Heart failure (HF) is a syndrome characterized by inadequate functioning of the cardiac capacities associated with disability. Its multiple etiologies and risk factors vary geographically, so it is proposed to know the clinical characteristics of HF in the Dominican Republic with the aim of suggesting improvements in health services.
Methods: 2,119 cases of patients with HF were identified in Hospital General de la Plaza de la Salud (HGPS) and based on this population size, a sample of 326 cases was calculated, based on a 95% confidence interval and a 5% margin of error. Based on the sample calculation, 378 cases were randomly selected. This oversampling was carried out anticipating the possible extraction of some cases in the final analyzes. After sampling, data was obtained.
Results: The study showed that the average age was 69 years. Diverse risk factors were evident predominantly arterial hypertension 87.5%, ischemic heart disease 50.4%, valvulopathies 40%, and diabetes mellitus 34.7%. Risk behaviors were also observed in which the use of tobacco predominated (23.7%), obesity (17.4%) and alcohol consumption (7.8%). The most frequent symptom was dyspnea with 84.7%. The average ejection fraction was 48%. The most frequent treatment was diuretics. The recorded mortality was 11.1%.
Discussion: This study evidences a high occurrence of behaviors and risk factors whose intervention and subsequent decrease could mean important changes to the prevalence of HF in the country.

Resumen
Human sexuality is an extremely important part of people's lives; however, for many of them it becomes a subject full of myths and taboos. This work aimed to explore the perception that couples with more than two years of living with sexual desire and attitudes had as a starting point to make visible the beliefs, accepted and rejected behaviors that may be influencing enjoyment or dysfunctionality of a coupled marital relationship. Questionnaires were applied to a sample of 52 people from the Puyo Canton. Variables were included such as: age, sex, time of relationship with the couple, how old she was when she had her first relationship, if she was a woman: situation in her reproductive cycle (menstruation, amenorrhea, menopause, pregnancy) and level of education. Bar charts, contingency tables and Chi square tests were used. The results showed that, in the field of desire, sexual attitudes and fantasy, men stated that they like sex more often than women and that they feel much pleasure or enjoyment when thinking about sex. On the other hand, women declared that they rarely or sometimes have sexual thoughts, observing a lesser motivation for sexual activity, whether due to work or financial problems in many of the cases surveyed. These results will reinforce in the workshops carried out, the issues that generated distancing in couples.

Resumen
The objective of the study was to determine the metabolic alterations present in schoolchildren with obesity who attend the Pediatric Endocrinology clinic of the Doctor Virgilio Cedano Clinic from January to June 2019, in the municipality of Higüey in the province of La Altagracia. A descriptive, observational, cross-sectional study with retrospective data collection was conducted, where 68 clinical records were evaluated during the period January-July 2019. At the end of the investigation, it is concluded that the main metabolic alterations found in schoolchildren with obesity were acanthosis nigricans, vitamin D deficiency and dyslipidemias. The female sex was the most frequent. 38% of obese patients had a family history of type 2 diabetes. Insulinorresistencia has 49% of the population studied. Most obese schoolchildren have central obesity. 48% of patients with central obesity have insulin resistance. Insulin resistance is present in most patients with metabolic disorders.

Resumen
The triage seeks to identify those who need immediate attention and those who can wait safely for a longer time. An observational, descriptive and prospective study was conducted, with patients attending the emergency service of the Dr. Robert Reid Cabral Children's Hospital in the months of September, October and November 2018, for a total of 32%, 21%, and 47 % respectively. The total number of patients recruited was 660, 54 % male and 46% female. Age of greatest attendance of 1-6 years old, 311 for 47%. The 4th level was the one with the highest attention, 219 for 33%. Highest hourly flow from 11:00 a.m. to 4:00 p.m. with a 5.8% and 6.1%. Response time level 1: 50 sec; level 2: 22 min with 5 sec; level 3: 41 min with 12 sec; level 4: 49 min with 35 sec; and level 5: 48 min with 53 sec. Conclusion: the response to care is affected by the lack of structured triage, a faster and more timely response to patients with levels 3 to 4, thanks to the high volume of patients who attend and not to those of level 1 and 2.
