Introduction: Caries diagnosis errors have become a public health problem. Objective: Verify the consistency in terms of decision-making between Brazilian dentists from the country's public health system and the International Caries Detection and Assessment System (ICDAS), and evaluate the influence of professional experience on the management of the decay. Methods: There are 80 dentists in the public health service of Recife city, Brazil. All dentists were invited to participate in this cross-sectional study. Forty dentists agreed to answer questions about their decision-making in relation to the treatment of occlusal caries in low, moderate and high-risk caries situations. The time of clinical experience, the use of methods for the assessment of caries risk and the lesion activity were also inquired about. The T-test was applied with a significance level of 5 % to compare the correlation between the International Caries Detection and Assessment System scores regarding the mean age of participants. Results: The disagreement between the dentists and the criteria used by the International Caries Detection and Assessment System for decision-making were mainly related to the sound surfaces or lesions restricted to the tooth enamel. It was thus verified that lack of consistency for the low-risk condition reached a score of 3 (32 %), while moderate (95 %) and high-risk (85 %) conditions lay at a classification code of 0. The time of professional experience was not considered to have interfered with decision-making on any scale, regardless of the risk condition of the patient (p> 0.05). Conclusions: A divergence in terms of decision-making between dentists of the public health system and the International Caries Detection and Assessment System could be observed, specifically regarding sound surfaces or surfaces with lesions restricted to the tooth enamel. Furthermore, professional experience was seen as not influencing the management of caries.

Introduction: Endodontic sealing cements should have dimensional stability to control contamination by microorganisms.Objective: Determine the dimensional stability of endodontic cement Adseal® under relative humidity conditions.Methods: An in vitro study was conducted based on International Organization for Standardization specifications (ISO) 6876:2001. Five cylindrical molds were used. The cements were subjected to relative humidity conditions and two cabinets were used to keep the temperature at 37 °C ± 1 ºC, monitored with a thermo-hygrograph. Measurements were taken in the 1st, 2nd, 3rd and 4th weeks with a stereo microscope. The Friedman test was used to evaluate dimensional stability with a significance level of 5 %.Results: Under relative humidity conditions, Adseal® displayed a slight 0.07 % (11 024.2 µm) expansion the first week, a slight -0.06 % (11 009.2 µm) contraction the second week, and greater length expansion above the ISO standard of 1.46 % (11 177.4 µm) and 2.76 % (11 320.6 µm), respectively, in the third and fourth weeks, which was statistically significant during the exposure period (p= 0.003). Regarding diameter and perimeter, an increase above 1 % occurred in the first and fourth weeks.Conclusions: Length expansion dimensional changes were registered for Adseal® cement during the first four weeks under relative humidity conditions, with greater changes above the standard in the third and fourth weeks. Regarding diameter and perimeter, an increase above the standard occurred in the first and fourth weeks.

Introduction: Developmental defects are due to congenital malformations, deformations or disruptions. Lip and/or palate fissures are congenital structural deficiencies caused by lack of conjunction between developing embryonic facial processes.Objective: Evaluate the quality of cleft lip and/or palate management guidelines.Methods: A descriptive study was conducted. Evaluation was performed of six cleft lip and/or palate management guidelines published in English and Spanish from 1990 to 2012. The search was both electronic (global level) and manual (local level). The electronic search included databases such as MEDLINE, PubMed, EMBASE and SciELO, whereas for the manual search written requests for guidelines were sent to public and private organizations that manage cleft lip and/or palate patients, such as the Military Hospital, San José Hospital, Operación Sonrisa (Operation Smile) and El Bosque University. For their selection, the guidelines were filtered by title and content. Before assessing the guidelines, the evaluators standardized them. Finally, evaluation of the guidelines was performed following the instructions in the AGREE II tool.Results: The Chilean clinical guideline obtained 100 % in domains 1 and 4, whereas the guideline from Saint Thomas University in Bucaramanga obtained less than 50 % in the six domains.Conclusions: None of the management guidelines for cleft lip and/or palate evaluated met all the quality criteria in the AGREE II tool.

Introduction: True periodontal pockets are an important reservoir of gram-negative bacteria which may exert an influence on distant sites in a variety of manners, bringing about systemic alterations such as diseases derived from atherosclerosis. Objective: Explain the theoretical foundations supporting the proposal of chronic periodontitis as a risk factor for ischemic stroke. Content: 1) Atherosclerosis and cerebrovascular disease, an overview of their pathogenesis. 2) The periodontal microbiota, bases for its association with atherosclerosis and ischemic stroke. Topic presentation: Inflammation is an element of vital importance in all the stages of the atherosclerotic process, be it its onset, progress or complications. By their nature, chronic immunoinflammatory periodontal diseases and the associated anaerobic subgingival infection, constitute a metastatic injury that produces and spreads inflammatory mediators leading to atherosclerosis and participating in the colonization and rupture of atheromatous plaques, which are processes involved in the pathogenesis of ischemic stroke. Final considerations: Better knowledge about the risk relationship between chronic periodontitis and ischemic stroke would allow a rational use of strategies for the prevention and treatment of cerebrovascular disease based on detection of inflammatory processes such as chronic periodontal conditions.

Introduction: The health-disease relationship in the elderly is directly influenced by social health determinants, and such influence includes oral health. Objective: Determine the behavior of social determinants and oral health conditions in the elderly. Methods: A cross-sectional descriptive study was conducted of 339 subjects aged 60 years and over from Consultation Office No. 5 of Plaza de la Revolución Polyclinic from May 2015 to January 2016. The study was based on interrogation and clinical examination of the oral cavity, prior informed consent by participants. The variables analyzed were age, sex, occupation, economic status, family structure and functioning, housing conditions, eating habits and oral health conditions. Results: The 60-69 age group was the best represented (53.4 %), with a predominance of the female sex. Only 27.9 % of the elderly people who had a regular job had good oral health conditions, and more than 60 % of the patients examined who reported a low economic status had fair oral health conditions. There was a predominance of functional families with oral health conditions evaluated as good or fair. 73.5 % of those with good housing conditions also had good oral health conditions. Elderly patients with good oral health conditions represented 82.4 % of those with adequate eating habits. Conclusions: The favorable behavior of the determinants analyzed reflects their positive influence on the oral health status of elderly people.

Resumen
The publications on Stomatology, in Cuba, appear in many magazines of the national territory, foreign and not precisely specialized in the subject. Researchers of the stomatological union have given themselves the task of showing the world their achievements through scientific publication. But, it is the responsibility of each disseminating entity to show the transparency with which the editorial process is carried out and to require the authors to demonstrate the same characteristics with their research. Hence the existence of editorial policies and standards for authors, which each magazine in particular has the responsibility to review and systematize according to current times.In September 2018, the SciELO Network Meeting was held in Sao Paulo, Brazil where, among other topics, the communication of research through open science, the transparency of processes and the opening of contents with a view were discussed. to promote the reusability and replicability of the methods. These precepts and others motivated the aforementioned network to modify its editorial and indexing policies for the journals that comprise it.The Cuban Journal of Stomatology joins the group of journals that welcome the new policies of the SciELO network and intend that their authors show their research with the excellence we advocate for this new year 2019.The new rules for authors are available in the Journal's Open Journal System (OJS) in Spanish and English and among other updates they reflect the modality of "Letter to the Editor", section of the magazine that was needed to be able to give participation to the readers and authors interested in offering opinions and comments about our publication.The challenge now takes us to the systematic, effort and learning of everything that can be done in order to achieve that the Cuban Stomatology has a disseminator of excellent research.

Introducción: Los dentífricos con ingredientes activos previenen la caries dental en niños. Objetivo: Determinar la actividad antibacteriana in vitro de tres dentífricos sin flúor y dos soluciones control frente a cepas estándares de Streptococccus mutans. Métodos: Se realizó un estudio transversal, prospectivo y experimental a doble ciego. Se utilizó agar trypticasa de soya con el método de difusión a 37 °C durante 24 h. Se observó el tamaño de los halos de crecimiento inhibitorio en cada grupo. El análisis de los datos se realizó con el software SPSS15, mediante pruebas estadísticas de corroboración de distribución gaussiana de Shapiro-Wilk, prueba de Kruskal-Wallis y de Mann-Whitney. Resultados: El diámetro de inhibición en gluconato de clorhexidina al 0,12 % fue de 26,69 mm (± 1,85), en el agua destilada de 6 mm (± 0) y para las pastas dentífricas de 6 mm (± 0) y 22,93 mm (± 3,39). Al comparar los diámetros obtenidos por la acción del gluconato de clorhexidina 0,12 % y del agua destilada con los dentífricos libre de flúor, sólo en uno de los casos se obtuvieron diferencias estadísticamente significativas (p< 0,05). Conclusiones: No todos los dentífricos para la higiene bucal del bebé estudiados presentan actividad antibacteriana frente a la cepa de Streptococcus mutans ATCC 25175.

Introducción: Dentro del marco de su actividad profesional el estomatólogo se enfrenta a diversas afecciones como la reabsorción radicular interna, cuyos casos avanzados pueden complicar el pronóstico del tratamiento endodóntico, debido al debilitamiento de la estructura dental remanente y las posibles afectaciones periodontales. Debido a que la boca está localizada en uno de los puntos focales de la cara, cualquier alteración en la apariencia estética puede provocar implicaciones psicológicas que pueden ir desde una simple forma de esconder el defecto hasta la más grande introversión. Objetivo: Presentar un caso clínico de reabsorción dentaria interna resuelto con implante postextractivo de carga inmediata. Presentación de caso: Paciente femenina de 31 años con reabsorción dental interna de incisivo central superior. Se llevó a cabo un adecuado diagnóstico y plan de tratamiento del caso, colocándose un implante transalveolar cargado de forma inmediata y la rehabilitación definitiva metalcerámica una vez concluida la fase de osteointegración. Conclusiones: Se consiguió una rehabilitación estética, funcional y biomecánicamente estable a largo plazo, que coincide con las expectativas del paciente, lo cual demuestra que cuando se cumplan los requisitos adecuados y la planificación sea exhaustiva, la implantología ofrece el tratamiento más avanzado en casos en que es inminente la extracción dental. A case of a 57-year-old female patient, with history of primary infertility, chronic kidney disease (CKD) stage V secondary to obstructive uropathy; in whom, weight loss studies documented metastasis of poorly differentiated adenocarcinoma without identifying its origin in a retroperitoneal biopsy. The patient evolved insidiously in the course of 16 years. Subsequently, she consulted for progressive dyspnea. Pleural fibrosis and persistently elevated erythrocyte sedimentation rate over the years was documented. The patient progressed to acuterespiratory failure, hydropic decompensation and died. The autopsy showed compatible findings with retroperitoneal fibrosis due to IgG4-related disease

Resumen
Meningoccocal disease is one of the leading causes of death in children and adults in emergency services. The presence of nonspecific symptoms as headache, fever and myalgias can cause delayed diagnosis, mainly due to a late start of antibiotics, thus producing an increasein morbidity and mortality and disabling potential among those who survived the disease, due to the long-term neurological damage. On the other hand, the presence of a petechial skin rash in a critically ill patient or with meningeal signs should remind us of acute meningoccemia, and the approach of these patients should never be delayed due to its high mortality. In the emergency room should always be considered the isolation from secretions by drops of those patients with suspected meningococcal disease or with a confirmed diagnosis, as well as adequate prophylactic antibiotic therapy must be start immediately to those who had direct contact with patient´s nasopharyngeal secretions.
