Starting from recent works devoted to the economy of esteem, this paper investigates the way in which esteem is demanded and distributed. Starting from the situation of its inequality of distribution related to the various types of social positions, it asks how this difference of attribution of recognition generates conflicts. Struggles for recognition can be interpreted as dangerous conflicts for the preservation of social relations because of the fact that they do not seem to admit the possibility of compromises and create situations characterized by the logic of “either/or” specific to identity politics. The paper submits this thesis to criticism, and provides a way to think about such struggles for recognition as constitutive of social integration.

This paper argues that the recent Latin American boom is the joint effect of the commodity price boom (primarily of mineral rather than agricultural goods) and exceptional external financial conditions. It has been reflected in a reduction in foreign indebtedness and increased international reserves, but not in fiscal austerity, and has reinforced the traditional specialization of the region in natural resource-based exports. Together with a slower growth of the workforce, it has generated a rapid increase in the employment rate and the diffusion of the benefits of the boom to a broader segment of the population, but it has not reversed the accumulated deterioration in job quality.

Introducción: La osteotomía de la tuberosidad anterior de la tibia (O-TAT) es una técnica quirúrgica que permite restablecer la alineación distal de la rótula en pacientes esqueléticamente maduros. Los objetivos de este estudio fueron evaluar los resultadosfuncionales y analizar factores que influyeron en el desarrollo de complicaciones.Materiales y Métodos: Se analizaron retrospectivamente pacientes con O-TAT tratados entre 2008 y 2016. Se documentaron datos demográficos y clínicos. Los resultados fueronevaluados según las escalas de Kujala y Tegner-Lysholm, y las complicaciones, con una adaptación de la clasificación de Clavien-Dindo.Resultados: Se analizaron 33 O-TAT en 29 pacientes (17 mujeres). Mediana de la edad: 18 años (RIC 2, mín.-máx. 14-39). Mediana de seguimiento: 49 meses (RIC 2, mín.-máx. 12-115). Mejoría en las escalas de Kujala y Tegner-Lysholm de 61 y 61,5 a 94 y 92,3, respectivamente (p = 0,001). La mediana de tiempo de consolidación fue de 8 semanas. Hubo 9 complicaciones (27%): 1 grado II (infección superficial) y 8 grado III (artrofibrosis, fracturas de tibia y fracturas de TAT, n = 2). La tasa de complicaciones fue más alta en los pacientes sometidos a desinserción de la TAT (51,5% vs. 11,1%, p = 0,029).Conclusiones: La O-TAT representa una técnica eficaz para tratar diversas patologías de la articulación patelofemoral en adolescentes y adultos jóvenes. Hubo complicaciones en un alto porcentaje de los procedimientos, sin que esto afectara el resultado final. El riesgo de complicaciones fue mayor en las osteotomías que requirieron desinserción de la TAT y la reconstrucción ligamentaria. ABSTRACTIntroduction: Tibial tubercle osteotomy (TTO) is a surgical approach that allows for the restoration of distal patellar alignment in skeletally mature patients. The objectives of this study were to evaluate functional results and to analyze the risk factors associated with complications. Methods: We carried out a retrospective analysis of patients subjected to a TTO between 2008 and 2016 and documented demographic and clinical data. Results were evaluated according to the Kujala Anterior Knee Pain Scale and the Tegner-Lysholm Knee Scoring Scale. Complications were evaluated with a modified Clavien-Dindo Classification of Surgical Complications. Results: We evaluated 33 TTOs in 29 patients (17 women) with a median age of 18 years (IQR 2, range: 14-39) and a median follow-up time of 49 months (IQR 2, range: 12-115). The Kujala and the Tegner-Lysholm scoring improved from 61 and 61.5 to 94 and 92.3, respectively (p=0.001). Union was achieved at a median of 8 weeks. There were 9 complications (27%): a grade II complication (superficial infection) and 8 grade III complications (arthrofibrosis, tibial fractures, and anterior tibial tubercle fractures). Osteotomies in which the tibial tubercle was completely detached had a significantly higher rate of complications (51.5% vs. 11.1%, p=0.029). Conclusions: TTO represents an effective approach for the treatment of several conditions of the patellofemoral joint in adolescents and young adults. In our series, a high percentage of the procedures presented complications, although they did not affect the final result. Osteotomies that involve complete detachment of the tubercle and those associated with ligament reconstruction have an increased risk of complications.

Background: Intra-articular treatment of knee osteoarthrosis (OA) with hyaluronic acid (IA-HA) is frequently used in Argentina, with different dosing forms and injection regimens. Our main objective was to provide real world data on the use and effectiveness of IA-HAMethods: In this observational retrospective cohort study we studied 1227 patients with knee osteoarthrosis treated with IA-HA 20mg/2ml weekly (76%-20mg group) and 40mg/2ml (24%-40mg group) according to usual practice intervals. Follow up was at 6 months and efficacy assessed by 5 point Likert scale and 50% or more response rate.Results: Median number of applications was 5 (20mg) and 3 (40mg). Both groups had a significant improvement in the Likert scale from baseline to 6 month. Median score improved from 4 (mean 3.77 ±0.69) at baseline to median of 2 (mean 1.99 ±0.76) at 6 months (20mg group, p <0.001) and from median of 4 (mean 3.65 ±0.65) to median 2 (mean 1.86 ±0.72) in the 40mg group, p <0.001. Proportion of patients with 50% or more 6 month response were 61% and 66% respectively. In the multivariate analysis, 50% response rate was associated with higher baseline score, younger age and shorter duration of disease. Only 1 patient in the 20mg group (0.1%) experienced a serious treatment related adverse event (septic arthritis) following injection.Conclusion: Our study in common clinical practice supports the effectiveness of IA-HA in step management of knee OA with clinical benefit observed at 6 month follow up.

Objetive: To evaluate the efficiency of the antibiotic cement rod (ACR) in the eradication of infection in infected humerusnonunion (IHN).Material and methods: We included 11 patients with IHN with a mean age of 48 years. The time between fracture-surgery was 25 months. The ACR was impregnated with vancomycin in 9 of de 11 cases. Follow-up was 54 months.Results: Methicillinresistant staphylococcus aureus (MRSA) was isolated in 5 of cases. All patients received antibiotics systemically for 7 weeks. Vancomycin was the most commonly used antibiotic. Time between ACR and reconstructive surgery averaged 56 days [confidence interval range (CIR) 47-98]. After debridement and implant removal, the residual space of the nonunion was measured with dichotomous variables and classified into two groups: group 1, < 2 cm (7 patients) and group 2, ≥2 cm (4 patients).  No significant differences were observed between the number of days in which the ACR was placed and the development of the SAMR as compared to other germs [48 days (CIR 45-75) vs. 73 days (CIR 56-149) p= 0.2002 Mann Whitney]. Nor were differences observed in the size of the defect in those who developed MRS or any other germ (p=0.242 Fisher). Reconstruction was performed with different techniques. Laboratory parameters were normal, cultures were negative. Fractures could be consolidated without infection recurrence.Conclusions: ACR is a good treatment option for a patient with an INH. The infection could be controlled in all of the cases, which allowed the secondary reconstruction of the nonunion

There are multiple techniques for spinal cord decompression in the thoracic spine, each with its advantages and disadvantages, and requiring different surgical skills. Recently, minimally invasive techniques have been developed, reducing morbidity rates and achieving good functional results. We present the case of a 64-year-old male with spinal compression symptoms, central disc herniation calcified at the fifth thoracic vertebra, which migrated to the lower end of the sixth thoracic vertebra. Diagnosis was clear for spinal cord compression. Partial posterior corpectomy of the sixth vertebra was performed with a minimally invasive transthoracic transpleural lateral approach and without additional fixation. The patient had a good outcome on follow-up, without progression of neurological symptoms or residual rib pain. Minimally invasive lateral approaches are valid techniques for the treatment of compression disorders of the thoracic spine, with low rates of morbidity and mortality, and a rapid recovery.

Introduction: Posttraumatic sequelae of the tibia is a common problem faced by orthopedics every day. The Ilizarov method, through careful planning, has achieved great results.Objective: To assess the outcome of the treatment of desires and posttraumatic discrepancies of the tibia treated with the circular fixator of the Ilizarov type.Materials and Methods: 13 patients were evaluated by clinical and radiographic criteria during a minimum follow-up of 24 months. The results were good and excellent in all cases, and bone consolidation was achieved in all patients.Conclusion: The Ilizarov method is useful and versatile to solve any type of post-traumatic sequelae of the tibia, without the need for grafts or bone substitutes.

Intraosseous disc herniation –or Schmörl nodes (SN)– are a herniation or prolapse of the nucleus pulposus of the intervertebral disc through the vertebral plate and into the adjacent vertebral body. They are usually associated with vertebral deformities, such as Scheuermann’s disease, or rheumatic diseases, such as ankylopoietic spondylitis. In general, they are spontaneous and asymptomatic findings, and there are only a few reported cases of symptomatic nodes. The etiology is supposedly related to a weakened spinal plate due to trauma or repeated stress. When the node is acute or recent, it can be difficult to differentiate a benign degeneration from a malignant infiltration or infection. In this paper, we discuss the unusual case of a painful Schmörl node in a man with no relevant history and a masked metastatic lumbar spinal tumor originated from pancreatic cancer. We performeda literature review.

Objectives: We sought to determine our 2 to 5-year survivorship, intraoperative fractures, subsidence and thigh pain rate in a consecutive, independent, one surgeon series of patients in whom a type-2B short stem was implanted.Patients and Methods: The first 100 type 2B consecutive short femoral stems implanted in 84 patients with a mean age of 47 years old were reviewed. Indications included age younger than 55 years (85 hips), or participation in impact sports in patients aging 56 to 60 years old (15 hips). Initial diagnosis was osteoarthritis in 82% of the cases. Median follow-up was 42 months.Results: Stem survival rate was 99% at 2 to 5 years for stem revision for any aseptic reason and 98% when infection was included. The mean Harris Hip Score improved significantly from 55 to 96 at final follow-up (p 0.02). There was one lateral cortex perforation, and three intraoperative calcar incomplete fractures (3%), only one of them required cerclage wiring and delayed weightbearing. No patient referred thigh pain at final follow-up. Only one case subsided 4 mm and then stabilized. The risk of revision was 1% at 2 to-5 years (95%CI 93.1-99.8%).Conclusions: Promising short-term results with this design were observed in this single surgeon, non-designerconsecutive series including the learning curve period using a type 2B short stem in young active patients, in which bone preservation is justified.
