  Reducing medical practice variation is an implicit objective of publicly funded healthcare systems. This paper describes hospital expenditure and hospitalization evolution in the Spanish National Health System (SNS), total and by certain characteristic conditions of effective, discretionary and supply-sensitive care, during the period 2004-2009 and 2010-2015, analyzing their trends and variability between health areas. During 2010-2015, the SNS notably increased its efficiency at the expense of increasing its activity and case-mix complexity while global spending was stabilized, although unwarranted variations between health areas did not decrease.   did not decrease.

    The text reviews some of the most important areas of public-private collaboration in health systems.   The recent evolution of the healthcare organization in Spain is outlined, analyzing and illustrating with examples, healthcare innovation. The emphasis of the article is placed onhealthcare management, both in terms of its quality and in the consideration of the impact of this quality on health outcomes. The limitations of public management, which influence the characteristics of healthcare management, are addressed, and the relative institutional deterioration of Spain is recalled in order to propose the measures that will allow improving the quality of its government. It concludes with two chosen big ways of improvement of healthcare management: the one centered in the public financing and the ones that pursue to make individually attractive what is socially convenient.

  Both, the health care system and the long-term care system (LTC) are important pillars of the Spanish welfare state. Although Spanish LTC system was born independent of the health care system, its own nature and objectives suppose a necessary integration, or at least, coordination between both. This strategy would save resources, allocate them optimally, reduce waiting times and improve the quality of life of people with long-term care needs. In this chapter the current situation is contextualized and a set of public policies are proposed to achieve progress in the welfare state.

  The arrival of numerous and diverse new technologies with innovative mechanisms of action under a pattern of incremental innovation demands a profound structural reform of the regulation of their authorization and financing. Many countries use economic evaluation as a fourth barrier in health financing decisions. In Spain, despite its explicit inclusion in the law, its role is still quite limited. Alongside with value-based approaches, innovative financing instruments are emerging, such as payment by results or price setting by indication.
