Shoulder pathology is one of the most common causes of care in Orthopedics, being caused by multiple entities such as instability, rotator cuff injuries, osteoarthritis, adhesive capsulitis, among others, which can present several signs or symptoms, and that will require comprehensive management. One of the fundamental pillars in the management of shoulder pathology is physiotherapy. Technological advancements have allowed the advent of devices that help us improve the physiotherapy process. Total knee replacement is a procedure with excellent outcomes as long as the objectives of alignment and ligament balance are met. Postoperative comfort and well-being are achieved through a suitable implant for each patient and the correct positioning of the prosthesis. The ROSA robotic system uses information collected before and during surgery, and provides the surgeon with the necessary tools to reproduce the specific anatomy of each patient. In this way, personalized implants are achieved based on the anatomical landmarks of each individual and planning based on specific biometric data.Level of Evidence: IV

Introduction: Our objective was to analyze the results of osteosynthesis treatment in patients with Vancouver type B1 and C fractures, evaluate complications, reinterventions and the mortality rate in this group. Materials and Methods: Multicenter, retrospective study. A database was established that included 53 patients with Vancouver type B1 and C periprosthetic femoral fractures treated with osteosynthesis from 2008 to 2021, who were evaluated in two high-complexity hospital centers. Results: The most used proximal fixation was bicortical screws and wire loops. The type of fracture according to the Vancouver classification correlated with a significant value in the use of interfragmentary compression screws (p 0.001), with a total of 13 patients (24.52%), 9 in Vancouver type C fractures. Mean consolidation was 4 months, with a mean Harris Hip Score of 68. Twelve patients (22.64%) had complications: delayed union (7 cases; 13.2%), failed osteosynthesis with fracture at the distal level of the stem (one case; 1.88%), one new osteosynthesis due to failure at the level of the osteosynthesis material (one case; 1.88%), and three deaths (5.66%). Conclusions: The management of periprosthetic femoral fractures is a complex and challenging issue. Osteosynthesis treatment is a successful method that requires the application of current principles of minimally invasive techniques that, together with stable proximal fixation, improve the chances of success

Introduction: Tranexamic acid (TXA) reduces blood loss and need for a transfusion after total knee arthroplasty (TKA). However, patients with a history of coronary artery (CA) stent placement might be at increased risk for thromboembolic complications. Materials and Methods: We performed a retrospective analysis of patients with a history of coronary stenting who had undergone primary and revision TKA and received preoperative TXA. A comparison was made with a group of patients without coronary stenting. The presence of any clinical or electrocardiographic changes of acute coronary occlusion, thromboembolic events (TEE), blood transfusion, and pre- and postoperative hemoglobin levels were analyzed. Results: 57 patients underwent 59 TKA surgeries (56 primary and 3 revisions) with a history of coronary stenting at least 1 year before arthroplasty. One patient presented symptoms of acute coronary syndrome and electrocardiogram (ECG) changes. There were no differences in the number of thromboembolic events. Only 1 patient received red blood cell transfusion in the control group. Relative bleeding was lower in the coronary group regardless of chronic use of aspirin and clopidogrel before surgery (2.09 vs 3.06 in the control group; p=0.01). In high-risk patients, TXA was not associated with higher TEEs. Conclusions: Although TXA seemed safe and effective in this database review of patients with previous placement of CAS; a larger prospective trial is warranted to confirm these results.

Irreparable rotator cuff tears are major injuries that can drastically affect the quality of life of the patients. Despite the complexity of the procedure and the high rates of re-tear, surgical repair is justified due to the lack of less aggressive and palliative alternatives. If a repair is not achieved, surgical alternatives are considered including tendon transfer, superior capsular reconstruction, or even reverse shoulder arthroplasty. Accordingly, arthroscopic repair must be performed in order to save the severely injured rotator cuff. The aim of this study is to summarize some of the arthroscopic techniques for repairing the so-called irreparable rotator cuff tears.

Objective: Displaced olecranon fractures mostly require surgical fixation. The most commonly used techniques are tension band wiring and plate fixation, but they are associated with high rates of discomfort due to soft tissue irritation. An alternative surgical option is fixation with high-strength sutures with intramedullary screws. The aim of this study is to evaluate the range of motion, bone healing and complications using this technique. Materials and Methods: Six patients with type IIA fractures with an average age of 43 years (range 24-60 years) treated with high strength suture and intramedullary screw between January 2020 - April 2021 were included. Post-operative range of motion, bone healing and complications were evaluated. All were evaluated up to 6 months postoperatively. Results: At the 6th postoperative month, the average flexion was 143º (range 90º-160º), average extension 19º (0º-55º), there was bone healing in 5 patients, and 1 nonunion with joint stiffness (complication 16.6%). There were no second surgeries. Conclusion: Simple displaced olecranon fractures treated with high-strength suture with intramedullary screws is a simple, reproducible and economical technique since it does not require a second surgery, as is the case with traditional fixation methods for this pathology.
