Resumen
Ocular trauma is usually severe, because it can potentially lead to loss of vision in one or both eyes, and even to the loss of the eyeball, therefore, they are a very frequent cause of urgency, both, in Primary Care Centers and Specialized Health Centers. The prognosis of many of these eye traumas depends on the haste and efficacy added to the attitude assumed by the first physician who has the first contact with the patient. Hence, the importance of the abilities of any physician in being prepared to be able to make a presumptive or positive diagnosis, and to be able to assume the responsibility of decision for each case, within all levels of the health care system. Such is the case of these two patients, who presented by themselves to the emergency department after an open ocular trauma. Orbital exenteration performed by the General Surgery Service, since the specialist staff of ophthalmology were not at the health center by that time. In order to limit infectious outbreaks, the procedure is prioritized through the dominant technique by the surgery treating emergency team. Initial management and decisions are especially important for further healing process, including the management of: orbital fractures, eyelid injuries, traumatic hyphema, trauma of the optic nerve and retina. Also, perforating wounds of the eyeball, ocular foreign bodies and chemical burns or radiant energy as well. Orbital exenteration is a treatment indicated in malignant and benign processes in which there are special risk of systemic infection, uncontrollable pain, irreversible loss of vision, deformity, tendency to diffuse or persistent infiltration and possibility of malignization.

Resumen
Chronic pain framed in complex therapy and partial clinic improvement is problematized in the medical consultation of neurosurgery at the Instituto Autónomo Hospital Universitario de Los Andes, there young patients manifest the need for "work rest", work adequacy or the possibility of a Request of "Disability" to avoid continuing in their work. With the purpose of exploring the tendency to separate the work environment as a patient's decision, justified this phenomenology in the information of the medical consultation, and at the same time its ethical implication, we performed primarily a prospective approach to patients employed in public administration, with chronic pain criteria, taken at random, both sexes, aged 25 to 55 years who attended between January and September 2016, in a qualitative and ethnographic research. In the context of contemporary Venezuela, patients of productive age, with need for change, present a dynamic that is maintained over time, is it a correct attitude of the patient of productive age, separated from one working environment to another in expectation of Change your health? Can the expectations of change in work activity represent an act of stoicism, hedonism, or biological necessity to face the pain? The decision-making process is carried out; In a free human, keeping intact its consciousness, perceptive capacity, understanding of the consequence of its acts, interpretive and response to its environment that starts with an ethical principle of autonomy in its decisions.

Resumen
The tumors of the oral cavity include several subsites: lip, anterior two thirds of the tongue, buccal mucosa, floor of the mouth, hard palate, gums, and retromolar trigone. They account for 5% of all neoplasms in men and 2% in women. The objective was to determine the prevalence of benign and malignant oral cavity tumors in patients treated at the Autonomous University Hospital of Los Andes, 20102014. The research method was structured under a quantitative approach, type of study descriptive retrospective cut, nonexperimental design, the sample consisted of 53 patients, the information was collected through the review of medical records and then was registered in a file of data collection. The results found that the benign tumor and anatomical location more frequent, is Pleomorphic Adenoma of hard palate 23.1%. The malignant tumor and more frequent anatomical location, well differentiated epidermoid carcinoma of the tongue base 33.3%. Patients with a malignant tumor diagnosis of 26.4% in the oral cavity report smoking. And alcohol users 13.2% are also associated with oral cavity cancer. It is concluded that there was no discrepancy in the frequency of benign and malignant tumors of the oral cavity, which indicates that they are approximately equally distributed. The female gender reports a greater number of oral cavity tumors. It is recommended to create prevention programs aimed at the screening of oral cavity injuries and to permanently train health personnel for the early diagnosis of pre malignant injuries of the oral cavity.

Resumen
Objective: to set up the advantages and disadvantages that laparoscopic cholecystectomy by single port (SP) offers above conventional technique (CT) in elective patients under the care of the General Surgery Department of the "Instituto Autónomo Hospital Universitario Los Andes" (IAHULA) during the period April 2013 - April 2015. Methods: It is a longitudinal case-control analytical study in which the advantages and disadvantages of laparoscopic cholecystectomy are explained by ithe intervention of 42 patients with gallbladder disease that required surgical treatment; 21 cholecystectomies were performed by the single port technique (Group of cases), and 21 cholecystectomies by conventional four-port laparoscopic technique (Control group) were also performed. Results: the overall mean age was 41.23 ± 15.5 years old people with a range of 17 to 82 years old. 90.5% of the cases (38 patients) were female. The overall operative time was 50.11 ± 16 minutes, with a range that oscillated between 25 to 90 minutes, with no statistically significant differences between groups (p = 0.122). Conclusions: In none of the cases of the SP group was there a need for conversion to multi-port technique, and conversion to open surgery was not necessary either. There were no transoperative complications, and in all cases of both groups transoperative bleeding was minimal, therefore it was not quantified. The Single Port Technique was reproducible with a high level of aesthetic satisfaction and short hospital stay, thus confirming the safety and efficacy of this novel technique.

Resumen
Acute, chronic, and other difficult to heal open skin lesions represent a public health problem worldwide and in Venezuela, being among the first places of morbidity and mortality, with a tendency to remain in time, with a deficiency of resources and attention predominantly as Dry cure. The efficacy of the Natural Bioactive Purpose (ABN) versus the Conventional Dressing (CA) was studied in cutaneous healing. A randomized, double - blind experimental study (Phase III) was designed in the outpatient network of Merida - Venezuela. The results were analyzed using Epidat 3.1, we calculated the sample size (99% confidence). Significant differences were found in ABN in cure rates and their comparison variables, with no side effects, allowing good quality of life. It was concluded that the ABN in comparison with the CA possesses significant clinical - therapeutic benefits, being considered a tool of choice for curing of open cutaneous lesions.
