Resumen
The Coronavirus (COVID-19) pandemic, typified as such in March 2020 by the World Health Organization (WHO), has exceeded the capacity of health systems to aid victims, and triggered a radical change in medical research, and in the monitoring of the enrollment for clinical trials in urologic fields around the world.
Last year, almost 90% of clinical sites closed patient enrollment, while at the same time, researchers around the world initiated almost 1000 COVID-19 clinical trials. This catastrophic pandemic has allowed us to expand our medical knowledge exponentially. The global urological community has created and published an infinity of scientific articles: establishing guideline reactions for diagnosis, treatment, and follow-up of the different urologic conditions across all areas of the field, reporting the experiences at urology services, and putting forward new strategies.
The Confederación Americana de Urología (CAU) has promoted international collaborative projects that have led to gaining insight into how the Latin American Urology Services faced the pandemic, including the challenges, strengths, and the areas of opportunity for urologic care. It also allowed us to increase the number and quality of publications. Also, we have created new virtual platforms and international networks to exchange our knowledge. We have as well transformed this social, economic and health crisis brought upon us by COVID-19, into a source of opportunities for the growth and promotion of research in Latin America.
Urologic patients, require researchers to work on favoring their goals. A collaborative network, the established and coordinated protocols, the safety of patients and researchers, assertive and constant communication, and effective technology use, are the essential tools to resume institutional investigation under these critical conditions.

Resumen
Introduction: Metastatic kidney cancer continues to be a therapeutic challenge for urologies and oncologies. With the new systemic therapies, the indication for initial cytoreductive nephrectomy is becoming increasingly controversial. The treatment of the primary tumor at a stage with systemic involvement continues to be a controversial issue.
Materials and Methods: By conducting a survey of Latin American hospitals, this publication aims to evaluate the indication for cytoreductive nephrectomy, the preferred surgical technique and the results obtained, estimating regional therapeutic lines. Online survey was made, based on the most important topics of discussion on cytoreductive nephrectomy and metastatic kidney cancer.
Results: A total of 20 centers participated on the survey, al of them affirmatively about the realization of cytoreductive nephrectomy. Most of them also offer systemic treatment. Regarding the surgical technique used, most used minimally invasive techniques.
Discussion: The international evidence about the role of cytoreductive nephrectomy in this context is varied and subject to permanent change. Individualized treatment, multidisciplinary team are fundamental for the approach of these patients.
Conclusions: The subgroup of patients with metastatic kidney cancer is conceptually heterogeneous. Cytoreductive nephrectomy continues to play an important role, sometimes leading role, in the management of selected patients.

Resumen
Background: Partin tables made from a 2007 cohort in a North American population are regularly used to predict the extent of prostate cancer after radical prostatectomy. There is no study that evaluated this table in the Ecuadorian population. Objective: To determine the usefulness in the prediction made by Partin tables in Ecuadorian patients with prostate cancer who underwent radical prostatectomy.
Materials and methods: A descriptive, cross-sectional, quantitative and retrospective study was designed, the latter being its limitation. The records of 119 patients with prostate cancer who underwent radical prostatectomy during the period from 2010 to 2020 were reviewed. The area under the curve (AUC) was calculated using the ROC curve, and the result for extraprostatic extension (EEP) was compared. , infiltration of seminal vesicles (IVS) and lymph nodes (INL) with the Partin 2007 cohort.
Results: The ROC curve for EEP showed an AUC 0.54 (0.43-0.65), for ISV 0.60 (0.44-0-76) with low sensitivity and specificity, while for INL an AUC of 0.73 (0.59-0.86) with acceptable sensitivity but low specificity.
Conclusions: Partin Tables did not predict pathological stages for EPP and ISV; however, for INL it can be used with caution.

Resumen
Objective: To analyze the evolution of laparoscopic surgery and the influence of technical elements, such as the use of bipolar grasper and barbed suture in surgical success.
Methodology: A retrospective review of medical files and surgical videos of laparoscopic procedures of a single surgeon was done. Descriptive statistics was performed in search of differences in surgical time and bleeding.
Results: A total of 140 procedures were included, with 89.3% performed in private hospitals, mostly radical prostatectomy (42.8%), radical nephrectomy (33.5%), partial nephrectomy (5.7%), and others. The average surgical time was 130.2 minutes, with an average bleeding of 430cc, with only 4 severe complications (2.1%), and a low mortality rate of 0.7%. The conversion rate to open surgery was 1.4%. The hospital stay was in average 2.4 days. The implementation of bipolar grasper and barbed sutures diminished the surgical time and bleeding.
Study limitations: Because this is a single surgeon’s experience in time, with diverse surgical procedures, a multicentric comparative study should be undertaken to confirm the results.
Originality: In Mexico, there are no studies that evaluate the use of technology in laparoscopic urologic surgery.
Conclusions: This paper reports on the evolution of the surgeon’s technique and experience, which have enabled an optimization in surgical times and postoperative outcomes associated to the use of the technology for dissection and the use of barbed sutures to improve results.

Resumen
Objectives: research what is the actual role of the laparoscopic approach in the treatment of benign prostatic hyperplasia, in daily practice of Latin American urologists.
Materials and methods: an online survey was conducted during the period June - September 202, send to all members of the Confederación Americana de Urología, through its official networks.
Results: laparoscopic adenomectomy is not the preferred approach for the treatment of prostates larger than 100g (4% vs 76% for open adenomectomy), most respondents never performed this technique (76%), however, urologists recognize that the laparoscopic approach has benefits over the open approach (64%).
Conclusions: although the laparoscopic approach has shown clear benefits in terms of surgical morbidity compared to the open approach, for most Latin American urologists, the technique has not yet been implemented in routine daily practice, being an unknown technique for most of respondents.

Resumen
Introduction: The terminology for adult neurogenic lower urinary tract dysfunction (ANLUTD) should be defined and organized in a clinically based consensus Report.
Methods: This Report has been created by a Working Group under the auspices and guidelines of the International Continence Society (ICS) Standardization Steering Committee (SSC) assisted at intervals by external referees. All relevant definitions for ANLUTD were updated on the basis of research over the last 14 years. An extensive process of 18 rounds of internal and external review was involved to exhaustively examine each definition, with decision-making by collective opinion (consensus).
Results: A Terminology Report for ANLUTD, encompassing 97 definitions (42 NEW and 8 CHANGED, has been developed. It is clinically based with the most common diagnoses defined. Clarity and user-friendliness have been key aims to make it interpretable by practitioners and trainees in all the different groups involved not only in lower urinary tract dysfunction but additionally in many other medical specialties.
Conclusion: A consensus-based Terminology Report for ANLUTD has been produced to aid clinical practice and research.

Resumen
Description of the clinical case: A 67-year-old man with history of radical cystectomy due to a muscle-invasive bladder tumor presented with an acute erythema and swelling in the left periorbital region accompanied by tenderness and decreased ocular motility. Initially, a periorbital cellulitis was suspected and empirical antibiotics treatment with initiated. As there was no improvement with the antibiotic regimen, it was decided to biopsy the lesion. The biopsy results were consistent with metastasis of bladder carcinoma.
Relevance: Bladder carcinoma represents one of the most common urological tumors, affecting predominantly males. The most frequent sites of metastasis are the lymph nodes, lungs, and bones; metastatic involvement of the orbit by bladder carcinoma is rare.
Clinical implications: Although metastatic involvement of the orbit by urothelial tumors is very rare, it is important to know the characteristics of the lesion in order to make a timely diagnosis and thus provide adequate treatment.
Conclusion: In patients with a history of bladder cancer who present periorbital cellulitis without improvement of symptoms despite adequate antibiotic treatment, orbital involvement due to bladder tumor metastasis should be suspected.

Resumen
Objective: To determine the association between the presence of genetic disease in the offspring and the paternal age.
Methods: We carried out a systematic review in: Medline (Ovid), EMBASE, The Cochrane Central Register of Controlled Trials (CENTRAL), from inception to October 2022. We included RCTs, Cohort, case-control and cross-sectional studies. Every study was evaluated according to eligibility criteria. Meta-analysis was not possible given the clinical and methodological heterogeneity.
Results: We identified 502 studies through the database search. Forty-five studies were selected for data collection and were included in the qualitative analysis. In this systematic review we did not find associations with anomalies such as Trisomy 18, Obsessive Compulsive Disorder, Tourette syndrome, schizoaffective disorder, adverse perinatal outcomes, hematologic disturbances, microcephaly, hydrocephalus, gastrointestinal disturbances, cleft lip with or without cleft palate, cleft palate, and genitourinary disturbances. In contrast, we found a tendency of association between older fathers and psychiatric disorders (schizophrenia, autism, bipolar disorder syndrome, intellectual disability), nervous system cancer and overall musculoskeletal congenital anomalies.
Conclusion: There is a tendency of association among the advanced paternal age and an increased number of birth defects regarding some specific conditions.
