Descripción: La terapia de piso pélvico es parte fundamental del proceso de rehabilitación de pacientes. Según el Colegio Americano de Médicos, técnicas como los ejercicios de Kegel, hacen parte de la rehabilitación del piso pélvico y son usadas en el manejo no farmacológico de la incontinencia urinaria femenina. en pacientes con múltiples condiciones clínicas que afectan la calidad de vida de los pacientes. Se pretende revisar la importancia de la terapia de los músculos del piso pélvico en la incontinencia urinaria, la gestación y el dolor pélvico crónico, así como enfocarse en el concepto mismo de la terapia. Relevancia: La terapia de los músculos del piso pélvico es una herramienta de bajo costo, con indicación en guías internacionales y que no reporta hasta la fecha, grandes efectos deletéreos para las personas que la practican como tratamiento de la incontinencia urinaria. Es de vital importancia, no solo hacer énfasis en la musculatura pélvica, sino que es necesario trabajar músculos del centro del cuerpo, donde se incluye la musculatura abdominal y espalda baja, para obtener mejores resultados para las personas en este tipo de tratamiento. Conclusiones: Esta revisión permite brindar conceptos claros y perspectivas al alcance del personal médico y fisioterapeutas en general.

    Description: Disorders of Sex Development (DSD) are congenital conditions characterized by a mismatch between external appearance (masculinity or femininity) and chromosomal constitution or gonadal sex. These manifestations are related to alterations at the level of gonadal development and the urinary-genital tract, and even the reproductive-endocrine system. Among the causes of said manifestations are those of genetic origin, which are caused by chromosomal abnormalities, particularly of the sexual chromosomes, or by the appearance of genes involved in the embryonic development of the sexual organs; as well as by anomalies that generate an interruption of the synthesis of specific hormones. Relevance: The group of DSD linked to alterations in the number of chromosomes includes the Klinefelter syndrome (47,XXY) (KS), and the Turner syndrome (45,X) (TS). Previous reports mention that sex chromosome aneuploidies directly impact genes, transcriptional factors, and epigenetic mechanisms that delay gene expression. Conclusions: There are few comparative molecular studies between patients with TS or KS in the literature. These studies are essential to understand the genetic processes that are related to the development of the pathologies of patients with these conditions, and thus, contribute to the improvement of the diagnosis, treatment and medical advice of TS or KS patients, directly impacting their quality of life. This article presents an updated review of DSD associated with sexual chromosomes, specifically TS and KS.  

We report the case of a 65 year-old man who attended his evaluation and follow-up for clear cell renal carcinoma, postoperative laparoscopic right radical nephrectomy pT3N0M0 in 2017. In his quarterly oncological follow-up, he attended with his control by abdominal-pelvic tomography in which local subhepatic recurrence was observed and an adrenal gland plus an occupying mass in the abdominal vena cava subdiaphragmatic level II of the Neves-Sincke classification, for which a laparoscopic metastasectomy with trombectomy was protocolized. With a preoperative hemoglobin of 17 ng/dl, and a creatinine of 1.75 ng/dl, blood glucose 11 ng/dl, the rest of the studies normal as well as a normal general urine test. The intervention course without intraoperative complications, bleeding calculated of 1400 ml which required a transfusion of 1 unit red blood cell, being discharged at 72 hours. The histopatologic result clear cell carcinoma Furhman 2 with thombous.

Urinary infection by Proteus mirabilis is complicated since this ureolytic bacterium generates large amounts of struvite crystals in urine. In this study, 54% of the urines with more than 25 struvite crystals per field had a positive urine culture for Proteus mirabilis, the majority also presenting an alkaline pH and abundant leukocyturia. In most cases, the presence of struvite crystals in urine is related to urinary infection by Proteus mirabilis. Therefore, urinalysis allows us to guide the diagnosis before the result of the urine culture.

Objective: To present our experience in Endoscopic Inguinal Lymphadenectomy (IEL) in the management of penile cancer with risk factors for metastasis and clinically negative inguinal nodes (cN0); to evaluate the results obtained in the long term and with those described in the literature review, proposing "ideal objectives" for this minimally invasive technique. Methodology: Between 2012 and 2016, patients diagnosed with penile cancer who met the criteria for pathology greater than pT1G2 and cN0 inguinal nodes underwent IEL. We recorded pre-surgical, perioperative, pathological and follow-up data. We analyze results and discuss the evidence described in the literature review. Results: In 15 patients with a mean age of 59 years, 23 IELs were performed (08 bilateral and 07 unilateral). The average operative time was 126 minutes; all cases with bleeding less than 50cc. The mean number of lymph nodes dissected per inguinal region was 10.4; in 3 patients, 1 metastatic node was found. There were no Clavien-Dindo III or IV complications. During the mean follow-up period of 89 months (range 72-120), no patient had disease recurrence or progression. Conclusions: Long-term evaluation in this group of patients has shown to be an effective, safe and reproducible alternative with optimal results in cancer control, prompt functional recovery and minimum intra and postoperative complications. The various publications coincide in affirming that it has advantages compared to the conventional one, fulfilling these three “ideal objectives” that we qualify as a trifecta, like the concept used in other urooncological surgeries.

Conocí a Javier Hernández-Pacheco Sanz siendo ambos residentes en el Colegio Mayor La Moncloa el año 1971, cuando él estudiaba Filosofía en la Complutense y yo había terminado recientemente la carrera y preparaba el doctorado. Entre las muchas anécdotas de aquellos años me viene a la memoria una charla-tertulia sobre fenomenología que les di a Miguel García-Baró y a él, ambos compañeros del mismo curso (no recuerdo si había alguien más); también asistimos juntos a alguna conferencia de Zubiri en la Caja de Ahorros de Arapiles; estuvo presente, asimismo, dándome ánimos, en el ejercicio oral de mis oposiciones a cátedra de Instituto. Era un gran amigo y un brillante filósofo, con quien departía frecuentemente sobre nuestras comunes inquietudes filosóficas.

By means of the resource that Rorty uses to argue that Heidegger has not left the Plato Kant canon, we are going to show that Rorty himself cannot get out of the platonic approach. For this purpose, our article is divided into two main parts. Firstly, we will systematize the inversion of Heidegger that Rorty left merely written down in "Overcoming the Tradition: Heidegger and Dewey", tracing the ontological and epistemological assumptions on which the pragmatist based himself to carry out his peculiar reading of Heidegger. Later, we will demonstrate through that the technique of inversion and recontextualization that Rorty uses with Heidegger can turn against him and not allow him to leave the Platonic matrix.

This article first exposes one of the central theses of Javier Hernández-Pacheco's thought: the thesis of the moral imperative that drives us to transform the world to turn it into an expression of freedom. And secondly, the discussion that Javier Hernández-Pacheco and the author of this article held about the compatibility of the end that is expressed in that moral imperative with the framework of nature expressed by the laws of physics is summarized.

Se presenta una interpretación de la filosofía de Javier Hernández-Pacheco, al hilo de su confesado aristotelismo. Sus planteamientos quedan resumidos en una filosofía de la reflexión, de la identidad y del deseo. El ser es concebido como sustancia y como actividad. No obstante, todos los planteamientos de Javier Hdez.-Pacheco se inscriben en una filosofía de la libertad, que es concebida como la forma más alta de reflexión y subjetividad. Esporádicamente se mencionan algunos puntos sobre los que el autor y Javier más abiertamente han discrepado: en particular, se discute la primacía de la identidad sobre la diferencia y la libertad.
