Resumen
Introducción: Las cardiopatías congénitas están relacionadas con defectos cardíacos que se desarrollan durante el embarazo y están presentes al nacer. El éxito de las intervenciones depende en gran medida de la educación de los padres/cuidadores sobre los signos y síntomas de alerta. Los cuidadores necesitan tener herramientas para hacer frente a las barreras del sistema de salud. Objetivo: crear un sistema como herramienta de seguimiento para los bebés con cardiopatías congénitas complejas que ayude a los padres/cuidadores y a los profesionales médicos a observar las variables fisiológicas del paciente y a brindar una retroalimentación oportuna y profesional a los padres/cuidadores desde la atención domiciliaria. Materiales y métodos:se presenta un sistema que proporciona apoyo a los pacientes que necesitan seguimiento de cardiopatías congénitas complejas mediante un programa de monitoreo en casa. El sistema consiste en una aplicación móvil que permite a los padres/cuidadores controlar las variables fisiológicas del paciente (peso, ingesta de alimento y oximetría). La información registrada en la aplicación se envía para ser evaluada en tiempo real por el equipo médico. Resultados: la aplicación proporciona un medio eficaz para enviar datos y comunicarse con el centro de control médico, además de ofrecer material educativo e informativo. De este modo, los padres/cuidadores reciben constantemente ayuda e información acerca del estado de su bebé. Conclusiones: la aplicación móvil podría ayudar a minimizar los costos de la atención médica y superar las limitaciones del seguimiento domiciliario, dando a los médicos la posibilidad de ver la evolución del paciente y dar recomendaciones oportunas.

Introduction: Nursing workload in intensive care units (ICUs) is related to efficiency and quality of care; however, there are no methods to quantify the nurses needed per shift in ICUs. Objective: To identify the most used tools to measure ICU nursing workload. Methods: An integrative literature review was performed using original articles in English, Spanish or Portuguese, published between 1991 and 2017 in the databases ScienceDirect, BVS, Scopus, and Embase. The search strategy was “nursing and workload and intensive critical or ICU and measure.” Duplicate articles or articles about intermediate care units were excluded. The quality of the articles was assessed using the Strobe checklist. Results: Thirty-six articles with a total of 19,036 patients were included; 50% (n=18) used NAS, 27.7% (n=10) used a combination of methods such as NAS, NEMS, TISS-28 or VACTE; 13.8% (n=5) used TISS-28, 5.6% (n=2) used video camera recording, and 2.7% (n=1) used NEMS to quantify the time spent by nurses in care. Discussion: There is currently no consensus on workload measurement methods in nursing. In this sense, more validation and comparison studies are needed to improve nursing care management in the ICUs. Conclusion: The most used tool to quantify workload in nursing is the Nursing Activities Score (NAS). Other tools identified were NEMS, TISS-28, and VACTE.

Resumen
Introduction: venipuncture is an attribution procedure of the nursing team and stands out among the essential technologies to ensure the survival and therapy of acute/chronic childhood comorbidities. Objectified to discuss the social representations of nursing professionals and caregivers about peripheral venipuncture performed in hospitalized children. Materials and methods: research outlined in the procedural and structural approaches of the Theory of Social Representations carried out in pediatrics at a general hospital in Minas Gerais in April/September 2018. Sociodemographic data were collected and treated by descriptive statistics; free evocation of non-hierarchical words analyzed prototypically and lexicographically and; in-depth individual interviews that underwent content analysis. All ethical and legal aspects are met. All ethical and legal aspects are met. Results: the representations about the puncture, according to the nursing professionals, were structured on the difficulties in performing the procedure and the insertion of the mother in shared care and for the companions had a negative impact on their psychological condition. Discussions: the nursing professionals' social representations of venipuncture were reduced to catheter insertion techniques, neglecting family-centered care, which in turn represented the feelings of seeing the punctured child. Conclusions: the representational approximations/distinctions identified between the subgroups about puncture in children contributed with reflections pointing to the need for a sociocultural and humanized resizing of nursing care.

Resumen
Objective: To describe the behavior of pregnancy and congenital syphilis in Colombia between 2012 and 2018 according to national notifications records. Methodology: An ecological exploratory study was conducted based on notifications to the public health surveillance system. The incidence rate and prevalence ratio were estimated for each department. Each estimate was established per ranges for 33 departments evaluated and expressed in grayscale maps based on rates and ratios evaluated. Epidemic curves by week of notification for pregnancy and congenital syphilis are also shown. Results: Arauca, Santander, Cesar and Caldas had the highest increase in pregnancy syphilis between 2012 and 2018 while Santander, Casanare and Amazonas had an increase in congenital syphilis during the same period. Other departments had a decrease in these events. Significant differences were found in case reporting from year to year in both events in the country (p<0.001). Conclusions: In Colombia, an increase in pregnancy syphilis was found while a variability with an increasing trend was found for congenital syphilis in recent years.

Resumen
Introduction: Oral diseases are associated with unfavorable social, material, and socioeconomic conditions. Women’s oral health is affected by social aspects, which increase health inequalities when compared to men. Objective: To evaluate the social gradient’s influence on the oral health of working women at a university in Santander, Colombia. Materials and methods: Analytical cross-sectional observational cross-sectional study involving 84 working women. Sociodemographic variables, oral health condition variables (presence of dental caries, periodontal disease, and edentulism), and social status variables were used to establish relationships. Chi-square test or Fisher’s Exact test, Student's t-test or Wilcoxon rank test were used, with a significance α≤0.05. Results: The prevalence of dental caries was 85.7%, periodontal disease 79.8%, and edentulism 40.5%. Age, ethnicity, marital status, educational level, work carried out daily, socioeconomic status, and responsibility for household finances were the social factors most frequently related to oral conditions. Conclusion: The social gradient did not register statistically significant differences when analyzed with oral diseases. However, it was established that women with a high social status had a lower burden of oral diseases, while women with a low social status had a higher prevalence of dental caries, periodontal disease, and edentulism.

Resumen
Introduction: aromatherapy consists of the use of essential oils in the prevention or treatment of various human conditions. In labor, it can be applied to relieve pain and anxiety. Objetive: identify in the scientific literature on the use of aromatherapy and essential oils in the management of labor; and to elaborate a hospital protocol, based on the findings in the publications, on aromatherapy and application of essential oils in labor. Materials and Methods: this is an integrative literature review developed in the LILACS, Cochrane Library and Pubmed databases. Original scientific articles published between 2000 and 2019 were included. The material collected was analyzed based on Laurence Bardin's thematic content analysis. Results: thirteen articles integrated the corpus of this research. From them, four categories of analysis emerged: 1- Aromatherapy as a strategy for pain relief in the dilatation phase of labor; 2- Aromatherapy as a strategy to reduce anxiety in labor; 3- Methods of administering essential oils in labor; and 4- The use of essential oils for the management of unpleasant symptoms and the progression of labor. Discussion:based on the results, a hospital protocol was developed on the use of aromatherapy in labor. Conclusions: aromatherapy is an adequate tool for humanized care in the management of pain and anxiety in labor, with no adverse effects documented in the literature surveyed.

Introduction: mineral and bone disorder (BMD) is a serious complication of chronic kidney disease (CKD) that increases risks for death from cardiovascular causes and impairs quality of life of affected patients. Objetive: to evaluate the prevalence of BMD in patients with CKD and the association between estimated Glomerular Filtration Rate (eGFR) and BMD indicators (calcium, phosphorus and PTH) in non-dialysis patients. Materials and Methods:sectional study of a two-year cohort of chronic renal adults and elderly patients on conservative treatment. BMD was identified by serum values of: PTH (> 150 pg/mL) and/or hypocalcemia (Ca < 8.8mg/dl) and/or hyperphosphatemia (P > 4.6 mg/dl). The statistical analysis used: Poisson regression; Student's T, Mann Whitney and Pearson and Spearman correlations with 5% significance level. Results:BMD prevalence was 54.6% (n=41) (95% CI: 43.45 - 65.43), more frequent in women, literate, elderly, non-drinkers, non-smokers, sedentary and white skin color. Correlations between P and PTH with GFRe were significant, inverse, moderate strength (p= <0.005 and p = 0.003; correlation coefficients = - 0.312 and - 0.379 respectively). Discussion: the findings of this study highlighted gaps in the monitoring of BMD-DRC by primary care, requiring a review of clinical practices. Conclusion: robust prevalence of BMD in the early stages of CKD was identified, in addition to correlations between increased phosphorus and PTH levels and worsening kidney function.

Introduction: the literature already points out that the family is often the main obstacle that prevents elderly people from experiencing their sexuality. However, there are no studies that identify the effects of sexuality on family functioning. Objective: analyzing the effects of sexuality on family functionality and on the quality of life of the elderly. Materials and methods: a cross-sectional study conducted with 692 elderly people between the months of July and October 2020 by self-administered instruments. Data were analyzed using the Kruskal-Wallis test, Pearson correlation and structural equation modeling. Results: the elderly with some degree of family dysfunction had a worse experience in sexuality and a worse quality of life. The affective relationships domain of sexuality was the only one to exert a positive effect, from moderate to strong magnitude on family functionality (PC=0.472 [CI95%=0.301-0.642] p<0.001). Quality of life, in turn, had a positive effect, from weak to moderate magnitude, in all domains of sexuality: sexual act (PC=0.339 [CI95%=0.190-0.488] p<0.001); affective relationships (PC=0.117 [CI95%= -0.041-0.275] p<0.001) and physical and social adversities (PC=0.150 [CI95%=0.074-0.226] p<0.001). Conclusion: sexuality among the elderly can be explored more frequently in health services, as it exerted positive effects on family functionality and quality of life in this population. It is expected that with the results of this study, there is an appreciation of the theme in care services and that sexuality in old age is explored with the elderly, especially in primary health care.

Introduction:Tracheostomy procedures in intensive care units are on the rise; however, they can lead to both perioperative and postoperative complications, with a variable incidence from 5 to 40% and even death in up to 1.4% of individuals. Despite this, few studies address causal concepts or mechanical and nonmechanical risk factors about this important topic. Objetive: To review the scope of the available scientific literature on complications of mechanical and non-mechanical origin associated with a tracheostomy. Materials and Methods:The research question and inclusion criteria were established to conduct the search in PubMed and EBSCO databases between 2015 and 2020. The PRISMA-ScR checklist was used in the present study as a methodological and quality guideline. Results:The most frequent complications were bleeding 61%, tracheal stenosis 28.5%, decannulation 23.6% (5/21) , stoma infection 19%, death 19%, and difficult tracheostomy tube insertion 19%. Regarding mechanical risk factors, only the use of the Bjork flap (OR=0.4) was identified as a protective factor. Among the non-mechanical factors, obesity (OR=5.15), tube diameter >6 (OR=2.6), and preoperative mechanical ventilation (OR=3.14) were found. Conclusions: It was possible to identify that the highest incidence of tracheostomy-related complications were bleeding, tracheal stenosis, accidental decannulation, and death; however, it is still unknown whether they originate from a mechanical or non-mechanical cause during intensive care management.

Introduction: Toxoplasmosis is a zoonosis prevalent in one-third of the world’s population, adversely affecting maternal-fetal health by causing varying degrees of damage to the fetus. Objective: To assess the seroprevalence of Toxoplasma gondii IgG and IgM antibodies and associated risk factors in first-trimester pregnant women in Cúcuta, Colombia, in 2018. Materials and Methods: Cross-sectional and correlational study in 111 women who voluntarily participated. Specific IgM and IgG antibodies were detected by luminescence immunoassay (LIA). Results: Total seropositivity for IgM and IgG was 19.8% and 35.1%, respectively, 11.7% was for IgM only, and 53.2% corresponded to the overall seronegativity rate for T. gondii. Risk factors (CI = 95%) such as undercooked meat consumption (54.1% of cases, OR = 1.8, p = 0.120), tap water consumption (48.6%, OR = 1.4, p = 0.421), and goat or cow raw milk consumption (39.6%, OR = 0.78, p = 0.553) were identified; in addition, living with cats (23.4%) was identified as a risk factor significantly associated with parasite seropositivity (OR = 2.8, p = 0.025). Discussion and Conclusions: Our findings showed a possible risk of primary infection in more than half of the pregnant population, given the seronegativity against the parasite. A considerable frequency of suspected cases of very recent infection was also found. In addition to being associated with a previously recognized risk factor, this fact suggests the presence of other dietary risks that should be addressed through prevention strategies during prenatal care and the need to strengthen event surveillance.
