Near-fatal asthma (NFA) is defined as the most severe form of asthma characterized by a refractory asthma attack with an arterial carbon dioxide tension (PaCO2) greater than 45 mmHg and altered consciousness, requiring mechanical ventilation. We reported the case of 40-year-old female patient, obese and asthmatic with irregular treatment who presented dyspnea accompanied by severe oppressive chest pain and loss of consciousness, with generalized cyanosis and severe shortness of breath, for which she underwent emergency endotracheal intubation and mechanical ventilation. Laboratory tests show decompensated respiratory acidosis, glycemia of 258 mg / dl and HbA1C of 7.94%; her diagnosing diabetes mellitus. Asthmatic patients with type 2 diabetes mellitus who have irregular treatment for both diseases are at increased risk of manifesting near-fatal asthma.

Autism spectrum disorder (ASD) is a neuropsychiatric disease, characterized by deficits in social communication, presence of restricted interests and repetitive behaviors. This review aims to address the different nutrients that can be included in the diet of patients with ASD in order to reduce the different signs and symptoms present in this disorder. Different bibliographic sources were reviewed, such as PubMed, MEDLINE, ScienceDirect, Embase, and SciELO, using the keywords "Probiotics", "Vitamin B", Vitamin C", "Gluten", “Omega-3" and “Autism Spectrum Disorder”. It was found that probiotics and gluten improve gastrointestinal symptoms and, in addition, like vitamins B6, B9, B12 and C, as well as omega 3, help improve neurobehavioral symptoms, language and social behavior of children with ASD.

Introduction. The use of preoperative magnetic resonance imaging (PMR) in patients with breast cancer is debatable, with little agreement on its usefulness above routine scans. The goal of this study was to assess comparative studies of individuals with non-advanced breast cancer who were treated with or without PMR. Methods. A search was conducted in MEDLINE/PUBMED, LILACS, and SCIELO for medical literature published between January 1, 2000 and March 31, 2021, and publications that satisfied the inclusion criteria were included. Results. There were 3,828 publications, 53 of which matched the inclusion requirements; the papers were examined, and the findings were summarized in tables. There were 46 single or multicenter retrospective and comparative investigations, three prospective, randomized, controlled trials, and four meta-analyses including patients with infiltrating ductal or lobular carcinoma or ductal carcinoma in situ. The comparative results were antagonistic and debatable; however, in the most relevant studies, it was demonstrated that PMR delays surgery; increases mastectomies and additional biopsies; increases detection of ipsilateral/contralateral disease that is not necessarily malignant; and no significant difference in the rate of loco-regional or distant recurrence was established. Conclusion. Conclusions. PMR in non-advanced breast cancer has debatable outcomes in terms of surgery type, reoperations, and progression-free survival, and it is still being studied.

Antiphospholipid syndrome (APS) is a systemic autoimmune disorder characterized by arterial, venous, or small vessel thrombosis and recurrent early pregnancy loss, fetal loss, or pregnancy morbidity in the context of persistent antiphospholipid antibodies. Characterized by the development of multiple thrombotic manifestations, simultaneously or within a short period of time; being portal vein thrombosis (DVT) a rare and serious clinical manifestation and a predictor of poor prognosis.The case of an elderly patient with abdominal pain and portal vein thrombosis associated with APS with antithrombotic treatment and analgesics of favorable evolution is presented.

Introduction: The pandemic caused by COVID-19 has affected people's way of life, and particularly, couple relationships. The main of this study was to evaluate the psychometric properties of the Couple Relationship Quality Scale (CRP-ASO) within the context of the lockdown due to COVID-19, in Peru. Methods: The CRP-ASO scale was applied to 499 adults (60 % women; Mage = 41.54 years, Sage = 13.48). The internal structure of the instrument was evaluated using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). Reliability was also estimated by calculating Cronbach's alpha (α) and McDonald's omega (ꞷ). Results: The results indicated a four factors structure: consensus, complicity-intimacy, satisfaction in the relationship and stability in the relationship, with high indicators of internal consistency. Conclusion: It is concluded that the instrument has satisfactory psychometric properties and can be used in similar samples.

Objective: Report the clinical case of Gastric Adenomyoma as etiology of pyloric syndrome. Clinical case: We present a 52 years old man who attended the emergency room due pyloric syndrome and who underwent a distal gastrectomy. The pathological report revealed a gastric adenomyoma with ulcerative lesion. Discussion: This pathology is an unusual finding, and the literature describe it with extremely low rate of malignant transformation. Conclusion: Gastric adenomyoma is a infrequent pathology and should be taken into account when endoscopic studies are not enough to determine the etiology of a pyloric syndrome.

Introduction: To improve the quality of care in Pediatric Emergency Services (SEP) it is essential to carry out measurements and surveys.  Purpose: Describe the resources and operation of the SEPs of public hospitals in Latin America.  Methods: Descriptive, quantitative and retrospective study. Survey conducted in SEP of Latin American hospitals with public funding and with PICU (2019). Data processed by REDcap and InfoStat programs. Continuous variables are presented as medians and ranges; categorical variables as percentages; productivity / resource ratios as a ratio. Univariate analysis was performed.  Results: Of 371 services in 17 countries, 107 (28.8%) answered the survey; 102 (95.3%) have an observation area and 42 (39.3%) have isolation rooms. The medians of annual visits / observation bed and daily visits / clinic were 4830.6 and 24.4, respectively. The number of beds increased by 74.1% in the seasonal peak, 6.1% of the consultations required hospitalization and 2.0% were assisted in the Resuscitation Sector. 37 (34.6%) SEP have> 80% of 27 items considered essential by the International Emergency Federation, 43 (40.2%) lack complete airway equipment and 74 (69.2%) perform triage. The median number of daily consultations is 38.4 / doctor and 35.3 / nurse. In 72 (67.9%) SEP there is a doctor and a nurse coordinator per shift, in 83 (77.6%) computerized data are handled, in 25 (23.4%) they do not perform diagnostic coding and in 16 (15% ) discharge summary is not prepared. 68 (64.1%) SEP use 5 protocols for critical situations, in 10 (9.4%) the medical personnel have teaching / research hours and in 43 (41%) there is a quality improvement plan.  Conclusions:The information obtained regarding resources and operation of public PEDs in Latin America reveals important gaps.

Introduction: Given the high consumption of salt in the population, the need arises to have a validated tool that measures the knowledge, attitudes and practices of salt consumption (CAP-salt).  Objective: To validate the questionnaire on CAP-salt.  Methods: Psychometric study of content validity through expert judgment. A total of 5 physicians from different specialties were counted. To calculate the degree of agreement between the expert judges, Aiken's V was used. As a decision criterion to keep an item, a value ≥ 0.7 was considered.  Results: In relation to clarity, V values greater than 0.80 were presented, in addition to a variation coefficient of less than 25%, therefore, none of the items was eliminated. Regarding coherence, they presented a coefficient of variation above 20% and V values greater than 0.71, confirming the decision not to exclude any of them. Regarding the relevance of the items, V values higher than 0.90 were also evidenced and neither did any of them present a value below the critical one.  Conclusions: The questionnaire has presented sufficient evidence of content validity in terms of clarity, coherence and relevance of the items through the aforementioned analyses. For this reason, it should be used to quantify the KAP of different population groups in the country.

Introduction:Pertussis tetanus and diphtheria, (DPT) are highly contagious diseases in children; worldwide vaccination schemes have been established for the prevention of these pathologies. However, different factors can lead to non-compliance with them. Objective: To determine the factors associated with non-compliance with the vaccination schedule against diphtheria,pertussis and tetanus(DPT) in Peru in2019. Methods:Analytical, retrospective study of secondary source based on data from 7 187 mothers between 15 and 49 years old regarding the vaccination schedule of their children older than six months of age obtained in the Demographic and Family Health Survey (ENDES) Peru 2019.The vaccination schedule of children over six months of age and the sociodemographic variables of the mothers were analyzed. A bivariate and multivariate analysis was performed using the chi square test for independence (p<0,050). Results:The sample included in the study was 7 187 mothers surveyed regarding the vaccination schedule for their children older than six months of age. The variables associated with the incomplete scheme by multivariate analysis were: low educational level (RPa: 1,19; 95% CI: 1,02-1,40), not having health insurance (RPa: 1,41; 95% CI: 1,23-1,60), wealth index poor (RPa:1,19; CI95%: 1,02-1,40), age of the mother under 20 years (PR:2,63; CI95%:2,06-3,35) and having two or more children (PRa:1,36; CI95%:1,19 -1,57). Conclusion: The index factors of wealth, mother's age, low educational level, not having health insurance and having more than two children are associated with non-compliance with the DPT vaccination schedule.

Introduction.Melanoma is a public health problem; it represents 4% of malignant skin tumors and is responsible for 80% of deaths from this type of neoplasm. Objective: Show the response to Temozolomide in patients with metastatic melanoma. Methods: Descriptive, cross-sectional study. The clinical response of patients with metastatic melanoma, managed with Temozolomide 200 mg/m2 once a day was analyzed for 5 days every 28 days. The risk factors analyzed were: histological variety, topographic region of the primary lesion, metastasis, ulceration, and Breslow. Descriptive statistics were used for normality Kolmogorov-Smirnoff, Student's t-test, and binary logistic regression. Results: There were 51 les, 47 met the criteria; 25 men, 22 women, mean age 54.45, minimum 22, maximum 85 years, Complete response was obtained in 3 (6.3%), partial response in 7 (14.8%), stable disease in 10 (21%) and disease progression in 27 (57.44%) patients. The presence of ulceration is associated with a higher Breslow index and, as a result, a higher risk of disease progression. Conclusions: Temozolomide as monotherapy is a treatment that presents low rates of complete response and partial response, showing better results in patients with lymph node metastases.
