Resumen
Introduction: The physiological changes to which older adults are exposed; are many times negative factors in their quality of life, especially in those who are in geriatric residences. Glycemia and glycosylated hemoglobin are useful as metabolic control parameters. Objective: To relate the basal levels of Fructosamine and Glucose in institutionalized older adults in geriatric residences in the municipality of Tiquipaya, September 2019. Methodology: Non-experimental, observational, prospective, cross-sectional study, with a quantitative positivist analysis approach, with a universe of 97 adults over 65 years of age, with a sample of 79 that met the inclusion and exclusion criteria, with a maximum acceptable error of 4.77%. Results: 71% (n=56) of the patients were female. The elderly dominated as an age group. It was evidenced that there is not a good agreement between basal levels of Glycemia and Fructosamine, through the statistical calculation of the Kappa index which was 0.023; Wilcoxon test 0.081; Pearson correlation test r= 0.281. The basal levels of Fructosamine have a high sensitivity and specificity of 82.14% (L.I. 62.42% - L.S. 93.23%) and 56.92% (L.I. 47.95% - L.S. 65.48%), respectively. Conclusions: There is no relationship between basal levels of Glucose and Fructosamine since they are parameters of metabolic evaluation in different time and one does not replace the other, therefore, the determination of Fructosamine should be implemented in addition to Glucose to monitor adult patients in the medium term.

Resumen
Pneumothorax and pneumomediastinum are very rare complications that occur in the context of COVID-19, but it is even rarer for both to occur at the same time. We present the case of a patient infected with SARS-COV-2 with a history of non-Hodgkin's lymphoma undergoing treatment. Chest tomography revealed pneumothorax associated with pneumomediastinum, in addition to subcutaneous emphysema. Chest tubes were placed as treatment.bilateral. Although pneumothorax and pneumomediastinum are considered self-limiting pathologies, in this case the patient died.

Castleman's disease is a diagnostic challenge that must be kept in mind in patients with lymphadenopathy, because it belongs to a heterogeneous group of lymphoproliferative disorders, it was described by Benjamin Castleman in 1956, it is a rare pathology, with an etiopathogenesis not yet established, in which several factors intervene such as: viral antigenic stimulation, VHH-8, HIV, immunodeficiencies, high levels of IL-6, that lead to an inflammatory process which triggers a diverse symptomatology, the definitive diagnosis is made by excisional biopsy.The clinical case of a 41-year-old female patient is presented, who comes to the emergency service with a clinical picture of approximately 1 year of evolution characterized by progressive weight loss, palpable adenopathies, evidence of a lymphoproliferative process and splenomegaly observed in the scan, HIVnegative, anemia and compromised general condition.

Resumen
This study aims to examine the frequency with which children from 1 to 5 years old use new mobile devices and their relationship with health. A systematic, retrospective review was carried out in Pubmed and Scielo; Of a total of 145 articles collected, 43 were included, whose inclusion and exclusion criteria were: publication from 2008 to 2021, without preference for any specific methodology, study variables (physical, psychological and social health), no animal studies, repeated works and without relevance for the study, the quality of the evidence was analyzed in relation to the p value and the 95% confidence index for each variable in question. The use of mobile devices >2hrs/day is associated with the risk of developing health disorders in children aged 1 to 5 years, proactive monitoring of guardians is needed.

Resumen
We present the case of a 64-year-old male patient with progressive symptoms of fever, dry cough, fatigue,diarrheal events and exacerbated dyspnea; who is treated in the first instance in the emergency area and then he is admitted to the Intensive Care Unit due to his serious health condition. During the initial approach,computed tomography of the chest, which shows pulmonary infiltrates with ground glass pattern, whose presence raises suspicion of an infection caused by the SARS-CoV-2 virus; same as laterit is shown to be exacerbated due to a co-infection with the Influenza A H1N1 virus. This case report presents the clinical, imaging and analytical evolution of a member of a population of patients who simultaneously attend clinical pictures of two epidemics: COVID-19 and Influenza A H1N1, mainly during the winter, and within the framework of a phenomenon known as a syndemic.

Resumen
Cranioencephalic trauma is a common cause of morbidity and mortality in the pediatric population, its secondary injuries such as epidural hematomas are frequently found, but in its location in the posterior fossa it only represents 5% and they are at high risk of presenting complications; In addition, simultaneous supra and infratentorial hematoma is very rare. In this paper we will present the high-risk surgical management of a patient with a large-volume supratentorial and infratentorial hematoma, describing the possible complications and observing the evolution. A supratentorial craniotomy and a sub-occipital craniectomy were performed to evacuate the hematoma. The patient recovered satisfactorily showing improvement in immediate postoperative period, being discharged recovering global function.
