Background: In Latin America and the Caribbean, both anxiety disorders and Obsessive-Compulsive Disorder (OCD) are among the most common mental disorders. In Paraguay, both disorders have been studied, but not in correlation. Objective: To determine the correlation of the dimensions of obsessive-compulsive disorder with anxiety factors in undergraduate medical students of the National University of Asuncion in 2019. Methods: Quantitative study, correlational-descriptive design, non-probabilistic chain convenience sampling, from April to May 2019, at the Faculty of Medical Sciences (Saxony campus). Universe made up of N=450 students and necessary sample of n=201. The Obsessive Compulsive Inventory-Revised (OCI-R) and the StateTrait Anxiety Inventory (STAIC) were applied. Results: 208 surveys were analyzed. Mean age was 20.875±1.76 years. The majority of respondents were female. A significant correlation (r=0.431) (p<0.05) was found between OCD and Anxiety in medical students. Conclusion: In the present investigation a positive correlation was determined for the dimensions of obsessivecompulsive disorder and anxiety factors; it is indicated that the quantitative increase in the score of these dimensions is accompanied by an increase in anxiety factors.

Métodos: Un total de 233 pacientes (108 hombres, 125 mujeres), 184 tenían urgencia hipertensiva/ 53 emergencia (54,44% /50,95% en mujeres) en el servicio de Urgencias durante 11 meses. Los pacientes fueron divididos en cinco grupos de edad según décadas a partir de los 40 años (media de 65,85 años) y un total de diez grupos según el tipo de medicación hipertensiva que utilizaban (IECA, ARA, BB, BCC, diuréticos, moxonidina y sus combinaciones). Resultados: Utilizando monoterapia antihipertensiva el porcentaje de urgencias hipertensivas fue del 100,00%, 50,00%, 41,66%, 33,33%, 21,05%. El uso de IECA + BCC + diuréticos disminuyó significativamente el número de urgencias al 0%, 18,47%, 21,05%, 25,00%, 33,33%; pero la adición de betabloqueantes disminuyó el riesgo. En general, 53 pacientes no tenían medicación (22,75%) y 68 de 233 pacientes eran fumadores (29,18%, 63,23% hombres), de los cuales 36 pacientes tuvieron una urgencia hipertensiva (52,94% de los fumadores). El mayor número de hipertensos no inmersivos se encontró en los pacientes que tomaban ARA, diuréticos y/o BCC, pero el menor número se mostró en los pacientes que tomaban IECA en combinación con moxonidina (-20,07%). El 22,02% de los fumadores no eran hipertensos inmersivos (-54,67% de los no fumadores). El odds ratio para obtener una emergencia hipertensiva en caso de que el paciente tuviera un perfil no inmersivo fue de 4,18 (IC 1,02 - 18,89, p < 0,05). Los pacientes que tomaban una medicación diferente (o ninguna) no tenían mayor probabilidad de desarrollar una emergencia hipertensiva (OR 1,21, p = NS).

Introduction: The COVID-19 pandemic has brought daunting challenges to the delivery of healthcare services globally in a variety of ways. So, the quality of care during the pandemic could have been affected. Objective: To determine the satisfaction of the external user regarding the quality of care at the Pampas Hospital (Huancavelica, Peru) in the context of a pandemic from January to March 2021. Materials and methods: Single group pretest-posttest design study. The sample studied was 114 users of the Pampas Hospital in the context of the COVID-19 pandemic. To assess satisfaction, the SERVQUAL questionnaire was used, made up of 22 questions on expectations, given to the user minutes before entering the consultation and 22 questions on perceptions, evaluated after the patient was seen. Results: The largest number of patients was female (66.67%), the predominant age range was 30 to 59 years (53.51%), most (92.98%) have Comprehensive Health Insurance. and the most frequent type of user are continuers (76.32%). An overall satisfaction of 56.20% was obtained, which basically depends on the dimensions of quality: security (60.75%), empathy (61.58%) and tangible aspects (63.38%). Conclusion: The percentage of satisfaction with the quality of the external user was 56.20%. You must continue to improve in the dimensions with good satisfaction and implement strategies for the levels of dissatisfaction.

Methods: A total of 233 patients (108 male, 125 female), 184 had hypertensive urgency/ 53 emergency (54.44% /50.95% in women) at the Emergency department during 11 months. Patients were divided in five age groups as decades starting from the age of 40 (mean 65.85 years) and a total ten groups depending on which type of hypertensive medication they were using (ACEi, ARB, BB, CCB, diuretics, moxonidine, and their combinations). Results: By using antihypertensive monotherapy percentage of hypertensive emergencies were 100.00%, 50.00%, 41.66%, 33.33%, 21.05%. Using ACEi + CCB + diuretic significantly decreased the number of emergencies to 0%, 18.47%, 21.05%, 25.00%, 33.33%; but adding beta blocker additionally diminished the risk. Overall 53 patients had no medication (22.75%) and 68 of 233 patients were smokers (29.18%, 63.23% male) of which 36 patients had hypertensive emergency (52.94% of smokers). The biggest number of non-dippers was found in patients who took ARBs, diuretics and/or CCB but the smallest number was shown in patients who took ACEi in combination with moxonidine (-20.07%). 22.02% of smokers were non-dippers (-54.67% nonsmokers). Odds ratio for getting hypertensive emergency in case patient had a non-dipper profile was 4.18 (CI 1.02 – 18.89, p < 0.05). Patients taking different medication (or none) did not have increased chance for hypertensive emergency development (OR 1.21, p = NS).

Ewing's sarcoma is a rapidly growing malignant tumor, with a prevalence of 1-5 cases per 1,000,000 inhabitants, its extraskeletal shape in the sinonasal cavity or paranasal sinuses is unusual. Objective: to describe the atypical location of this neoplasm and the importance of achieving a timely diagnosis. Female patient, with a mass in the right nasal cavity of two months of evolution, headache and epistaxis. With asymmetry in the right orbital region and deformity of the nasal septum, paranasal sinuses with symptoms of obstruction. Tomography revealed a mass that invades the paranasal sinuses. The biopsy showed Ewing's sarcoma. It was confirmed with CD99. The patient received chemotherapy and a surgical resection plan, however she died. The timely diagnosis and treatment of Ewing's sarcoma in the sinonasal cavity should not be based solely on clinical evaluation, it requires a tomographic, histopathological, immunohistochemical and, if possible, cytogenetic examination

Urothelial carcinomas are the 4th most common tumor. They can be located in the lower urinary tract (bladder and urethra) or in the upper urinary tract (renal cavities, renal pelvis, and ureter). We present a series of 3 cases, with a picture of macroscopic hematuria, documenting urothelial tumors, performing in these cases a lumboscopic approach to the kidney and an endoscopic resection of the bladder rim. Lumboscopic nephroureterectomy was performed with transurethral endoscopic bladder rump resection. The 3 patients were discharged on the second postoperative day without complications and in the three cases the bladder rim without evidence of tumor activity, and currently under surveillance with excellent oncological results. Lumboscopic nephroureterectomy with endoscopic resection of the bladder rim is a reproducible technique in experienced hands, with the same oncological results as the open approach.

Introduction: Foeniculum vulgare is a medicinal plant of great importance for its properties and nutritional value.Fennel extracts possess therapeutic properties against endocrine changes, psychological disturbances, discomfort and symptomatology present in the vital cycle of women. Objectives: analyze information on the different properties and therapeutic effects of F. vulgare in the different stages of the life cycle of women. Methodology: A critical and analytical search of the topic was performed ,by means of controlled language compiled from MeSH and DeCs in scientific meta-search engines. Results: The development of this review was feasible due to the variety of freely available international literature, studies and clinical trials of information relevant to the topic. Conclusion: This plant can be used as a treatment, prevention and as a source of nutrients; and its extracts have an important effect on the vital cycle of the woman depending on whether she is in menstruation, menopause or postmenopause.

Subacute systemic lupus erythematosus (SLE) represents 10% of all cases and is rare in its psoriasiform variety. We present the case of a 22-year-old female patient with a previous diagnosis of SLE and lupus nephropathy 2 years ago, who reported a clinical picture of +/- 2 months of evolution after the suspension of mycophenolate mofetil. Table characterized by the progressive appearance of discoid, crusty and scaly dermal lesions that involve approximately 80% of the body surface accompanied by foamy urine. Physical examination: bodily injuries respecting palms and soles, painful on acupressure. Capillary filling> 2 seconds. Laboratories: leukocytes 5930 and Granulocytes 90%. General urine test: infectious. This is subacute psoriasiform cutaneous SLE. The treatment was antibiotic, immunosuppressive and antihypertensive. Hospital discharge with mycophenolate and ciprofloxacin.

Mucopolysaccharidosis III is a rare disease, with an incidence of 1 in 70 000 live births, it is the most frequent within the group of Mucopolysaccharidosis and is caused by a defect in the heparan sulfate metabolism pathway. It is characterized by affecting the central nervous system in greater depth, the patient has a normal development until approximately 1 to 3 years of age and later begins with progressive deterioration, courses with developmental delay, behavioral alteration and sleep disorder, adding motor deficits and infectious pictures, culminating in a state of prostration. Life expectancy ranges from 20 to 30 years, although it depends on the phenotype, and the main cause of death is pneumonia. Definitive diagnosis is achieved by genomic tests and enzymatic assay. It does not have curative treatment, only palliation and support in the face of the complications that it develops.

The Human Immunodeficiency Virus (H.I.V.) in advanced stages confers the carrier susceptibility to develop opportunistic infections. Little is known about COVID 19 in patients with HIV infection, and although the majority of patients in this group coincide with risk factors for poor prognosis when coinfected with COVID 19, very few cases have been reported referring to the subject. In this article we present a clinical case of a pediatric patient, with no significant pathological history, which after further studies went on to a diagnosis of HIV-AIDS / COVID 19.
