Resumen: Se presenta el caso de una paciente obesa inmunodeprimida que presentó una sarna costrosa. Luego de la sospecha clínica se confirmó el diagnóstico mediante ácaro test. La paciente presentó algunas complicaciones asociadas a su condición general, como sobreinfección de sus lesiones cutáneas, epistaxis e insuficiencia renal aguda, que fueron tratadas. La sarna costrosa fue tratada con ivermectina oral con dosis de 15 mg (200 ug/kilo de peso ideal según la talla), los días 1, 2, 7, 8 y 15, obteniendo una excelente respuesta terapéutica.

The Harlequin syndrome is a rare neurological condition, most of the time benign. In general, most cases are idiopathic (although there are secondary causes). This condition consists in the appearance of unilateral facial erythema and hyperhidrosis triggered by exercise, heat, emotions or stress, which occurs due to a contralateral dysfunction of the sympathetic vasodilator and sudomotor pathway. We present the case of a 49-year-old male patient, with no relevant morbid history, who reports a medical history of 15 years of evolution, characterized by the appearance of erythema and excessive sweating of the right side of his face, which ends abruptly in the middle line of the face, triggered by exercise and heat during the summer. While the left side presents anhidrosis without erythema.

Los nevi azules son proliferaciones melanocíticas dendríticas benignas a nivel dérmico, congénitos o adquiridos, debido a un defecto migratorio embrionario de melanocitos a partir de la cresta neural. Se manifiestan clásicamente como una pápula, nódulo o placa de color azul o azul-gris. Muchos subtipos histológicos se han descrito, siendo los más comunes el nevus azul común, nevus azul celular y nevus azul combinado. Las formas esporádicas incluyen al nevus azul lineal, eruptivo, agminado y con satelitosis. La dermatoscopía característica muestra un patrón de pigmentación homogéneo monocromático azul o azul-grisáceo, con ausencia de otras estructuras. Sin embargo, se han descrito también patrones de pigmentación dicromáticos y multicromáticos, además de estructuras tales como red de pigmento, puntos, glóbulos, proyecciones radiadas, pseudópodos, áreas cicatriciales blanquecinas, patrón vascular y rosetas. El diagnóstico diferencial de los nevi azules incluye lesiones melanocíticas y no melanocíticas, benignas y malignas, destacando entre ellas el melanoma. Se presenta el caso de un paciente de sexo masculino de 30 años, portador de un nevus azul celular con cambios de rápida evolución, con desarrollo de lesiones satélites y un aspecto dermatoscópico sugerente de malignidad, simulando un melanoma.

Bullying is a phenomenon defined as a form of intentional aggression that includes verbal, physical or psychological acts that occur in a relationship with other people in a repeated, systematic and sustained manner over time. Most of the studies focus on bullying and its consequences in the normal school population, however, there is little exploration of bullying that is the result of a communication disorder. This is why the work of speech therapists inserted in educational settings is not clear in explaining the guidelines for therapeutic actions that favor the prevention of this phenomenon. The purpose of this research is to know the main conceptions that speech-language pathologists have about bullying in people with communication disorders at school age. With this objective, an investigation was carried out with quantitative and qualitative methods, mediated by surveys and in-depth interviews, respectively. It was found that more than 65% of bullying between people within educational establishments is related to children between 9 and 12 years of age with communication disorders. Regarding the self-perception of the speech-language pathology role in the issue of prevention and/or intervention in bullying issues, the concepts of favoring aspects of language and/or speech in children who have difficulty emerge, raising awareness of the actors involved, the reinforcement of self-esteem and interdisciplinary teamwork. Completing the analytical treatment of the interviews, it is established that speech therapy would play a fundamental role in addressing bullying in populations with communication disorders, being the professional who acts directly on communicative health.

The aim of this research is to evaluate the behavior of the oral diadochokinetic parameters in first grade students, who belong to two different sociocultural environments and have different phonetic-phonological performance. To reach this objective, the oral diadochokinetic parameters were evaluated through monosyllabic repetition of [pa], [t̪a] and [ka] using the syllable emission method in a specific time. The syllable emission analysis was carried out through the Motor Speech Profile program, which provided with measurement values for five diadochokinetic parameters (DDKavp, DDKavr, DDKcvp, DDKjit, DDKcvi). On the other hand, the phonetic-phonological performance was analyzed through the Classification of Phonetic and Phonological Adjustments (CLAFF). The results showed differences between both sociocultural groups only for the parameters DDKcvp/%/ y DDKjit/%/, while the other diadochokinetic parameters behaved in a similar way. The low sociocultural children group had a higher variability in the DDK parameters and a higher percentage of disturbances. Significant relations could be seen between some phonetic-phonological adjustments and some diadochokinetic parameters.

Previous studies, conducted in different countries (e.g. Australia, Canada, India, Republic of Ireland, USA, UK), have shown that speech-language pathologists (SLPs) use nonspeech oral motor exercises (NSOMEs) to treat speech sound disorders (SSDs), bringing attention to the substantial debate regarding the clinical effectiveness of NSOMEs. The aim of the present study was to investigate and characterize the use of NSOMEs by Portuguese SLPs in the intervention of SSDs, and to analyze the evidence that supports it. To do so, SLPs who provide therapy to children with SSDs were invited to complete an online questionnaire, based on a previous survey conducted in India by Thomas and Kaipa (2015). A total of 184 participants responded to the survey; 93.5% reported knowing about NSOMEs, 78.5% used NSOMEs in their intervention for SSDs, and 80.2% considered them effective in treating SSDs (89% indicated that their knowledge about NSOMEs was acquired through graduate and post-graduate courses; 98.5% reported that they used NSOMEs to improve the motor function of the articulators). This study offers an overview of Portuguese speech-language pathologists’ reported use of NSOMEs as part of the intervention of speech sound disorders in children. Many of the participants in this study reported that they did use NSOMEs in SSD treatments, regardless of the lack of evidence to support their use in this context. Furthermore, the results show that the percentage of SLPs in Portugal using NSOMEs is similar to those found in the USA, UK, Canada, and India, but different from those in Australia and Ireland.

Speech therapists’ work in educational contexts is essential for students presenting special educational needs. However, little is known about the influence of the amount of time granted by educational policy in speech therapists’ professional practice. The present study did the first analysis in Chile of the experience of speech therapists with over 20 years of professional experience regarding the amount of time defined in public policy for speech therapists’ practice carried out in educational environments. A qualitative study with a constructionist-narrative focus was done. Five semi-structured interviews were carried out with speech therapists who had over 20 years’ professional educational experience via an inductive-type content analysis. Results showed that speech therapists’ practice presents challenges arising from administrative, organizational and practical aspects emerging from a public policy which does not clearly define time assignment and its distribution in the functions pertaining to speech therapists’ educational role. The contribution of this pioneering research on the subject is that the speech therapists interviewed suggest changes to the policy implemented on the basis of the adjustments they have made in their professional practices after 20 years of work experience. It is necessary to continue research on the exercise of the role of speech therapists in educational contexts in order to improve professional practice and contribute to the educational policy implemented.
