Resumen
Introduction: Idiopathic normal pressure hydrocephalus is a clinical complex characterized by impaired gait, urinary incontinence and deterioration of mental status. Currently, there is not a standardized diagnostic scheme. The endoscopic third ventriculostomy began to be used a few years ago due to the high rate of complications of the derivative systems, but the prognostic factors of its success have not yet been clearly defined, and its indication is questioned. Objective: To describe role of endoscopic third ventriculostomy in idiopathic normal pressure hydrocephalus. Methods: A literature review was performed in PUBMED and preprint servers BioRxiv, MedRxiv and preprint.org. Articles from the last 15 years were selected without the application of language filters. The following descriptors Normal pressure hydrocephalus AND Endoscopic third ventriculostomy were used. Only those articles with the full text available were selected. Results: A total of 430 articles were identified. Ten investigations about endoscopic third ventriculostomy in idiopathic normal pressure hydrocephalus were analyzed, 8 were retrospective studies with 62.5% of them with a sample of less than 15 patients, in addition to two cohort studies. Conclusions: Endoscopic third ventriculostomy has been relatively successful in treating carefully selected patients with idiopathic normal pressure hydrocephalus. It is a safe and effective procedure, aimed at improving intracranial compliance through the restoration of cerebral pulsatility, and its influence on the dynamics of cerebrospinal fluid. Resumen
Introduction: At the present time it exists, a parallel global epidemic to the pandemic of obesity; given by normal corporal weight and high levels of general adiposity. Their identification like phenotype of obesity allows predicting risks that can affect the development of the pregnancy. Objective: To determine the anthropometric and metabolic features those distinguish to the obese normal weight pregnant and their relationship with the newborn size. Methods: An observational analytic traverse study was conducted. Of a population of 526 seemingly healthy pregnant in optimal reproductive age, a sample of 358 metabolically healthy ones was obtained, that was stratified in 163 normal-weight healthy (HNW) and 195 obese normal-weight phenotype (ONW), in those anthropometric, metabolic and ultrasonographic variables was studied and the newborn condition. Theoretical, empiric and statistical methods were used. Results: The phenotype ONW in pregnant showed superior securities of WC, Index WC/H, % BF, Sum of tricipital and subescapular skin folds and of the LAP. In them the subcutaneous and pre-peritoneal fats were superior and they had bigger frequency of big births for the gestational age. Conclusions: The phenotype ONW in the pregnant is distinguished for anthropometric, metabolic and ultrasonographic features of central obesity and it is associated with the Big babies for the age gestacional at birth condition. Introduction: The Triage method of classification, selection and evaluation used in emergency areas, allows patients to be ordered according to the pathology prioritizing its severity and life-threatening situation. Objective: To analyze the triage in patients who arrive at the Emergency Area of the Specialty Hospital del Sur Delfina Torres de Concha. Methods: A cross-sectional, descriptive and quantitative study was carried out to analyze the triage process of the emergency service at the Hospital del Sur de Esmeraldas. For this, we worked with a population of 235 users who attended the emergency service during the months of February and March 2021, framed in the inclusion criteria such as belonging to the adult stage >18 years. The variables studied were age, sex, symptoms, diseases and triage levels: I. Red Color-Immediate Attention; II. Orange Color-Very Urgent; and III. Yellow Color-Urgent. Results: The study population was made up of 62.12% female and 37.87% male, the most frequent ages were that of the groups of 36 to 45 years with 49.78%. Among the main symptoms, headache appears (39.57%) and the most frequent disease was arterial hypertension (37.02%). Regarding the classification, it was obtained that almost half of the users were classified between the colors red and orange. Conclusions: The users who were treated in the emergency room presented pathologies such as arterial hypertension, with symptoms of headache and almost half of them were classified in the colors red and orange. Resumen
Introduction: In recent years, the definition of acute coronary syndrome has encompassed the different forms of presentation of acute ischemic heart disease. Despite the current therapeutic possibilities, it still presents a high morbidity and mortality and there are no laboratory tools to suspect complications early. Objective: To determine the value of the changes in the concentration of creatinine, potassium and glycemia as predictors of adverse events of acute coronary syndrome in patients admitted to Hospital Universitario Manuel Ascunce Domenech in the period from October 2017 to October 2018. Methods: A analytical cohort study. The sample consisted of 124 patients. A form was made where the data were collected from the medical records, the variables were: age groups, sex, skin color, diagnosis, adverse event, creatinine, potassium and glycemia. Results: The age group over 60 years old, male sex and white skin color predominated. The main adverse events were arrhythmias and left ventricular dysfunction. Elevated creatinine was associated with left ventricular dysfunction with heart failure and acute pulmonary edema, hyperkalemia was associated with potentially fatal arrhythmias that degenerated into asystole arrest and death. Hypokalemia was associated with arrhythmias and hyperglycemia with the recurrence of the ischemic episode. Conclusions: All the patients with normal values of creatinine, potassium and glycemia without complications and their usefulness as predictors of adverse events of acute coronary syndrome was demonstrated. Introduction: The AstraZeneca vaccine against COVID-19 is a current issue as it is one of the most widely available vaccines in the world to deal with this disease. However, it presents clinical manifestations, likewise other vaccine, that range from mild to severe, presenting differently in each inoculated person and that, on certain occasions, generate mistrust due to its short-term adverse effects. Objective: To determine the clinical manifestations after inoculation with the AstraZeneca vaccine against COVID-19. Methods: A descriptive study with a cross-sectional design with a quantitative approach was carried out in the Emilio Lorenzo Stehle School in Santo Domingo de los Tsáchilas-Ecuador, with a population of 23 teachers who participated with their informed consent. The data collection instrument was a questionnaire which consisted of seven questions formulated according to the research variables. Results: The clinical manifestations after vaccination with AstraZeneca with the highest incidence were muscle pain, headache and fever. It is worth mentioning that there are people who did not have any symptom. In addition, a point of concern in a minimum percentage is the severity of the clinical manifestations that were mostly treated with paracetamol. Conclusions: The manifestations were of mild intensity, mainly in female teachers, with symptoms lasting up to three days, especially using paracetamol to alleviate discomfort. Introduction: The renal chronic disease affects around a tenth part of the worldwide population and the most used analytical methods for its diagnosis have a low sensibility and precision. Objective: To determine cystatin C's concentrations for evaluation of glomerular masking in pre-dialytic phases of the renal chronic disease. Methods: A descriptive, cross-sectional study was carried out. The study universe was made up of 102 patients with chronic kidney disease in stages from one to four who were treated in an outpatient department of the Nephrology service. The sample was 81 patients who did not present the modifier factors of glomerular masking, creatinine and cystatin C related with the renal chronic disease. Results: The patients older than 61 years predominated. The cystatin C augmented in more patients than the creatinine. The cystatin C augmented equally in both sexes and the creatinine more in men. The average of the results moved away over the average of the value of reference in the cystatin C and the creatinine did not present normal values in patients with renal chronic insufficiency. The equations that they use cystatin C were better forecaster of damage renal. Conclusions: The cystatin C was the best predictor of renal damage. Introduction: Current studies have experienced that there is a high prevalence of comorbidity of mental disorders in burned patients. In Cuba, published research on this topic is scarce. Objective: To determine the prevalence and associated variables of comorbidity of mental disorders in burned patients admitted to the Vladimir Ilich Lenin University General Teaching Hospital. Methods: A study was carried out from the quantitative, descriptive and cross-sectional approach. The universe consisted of 140 burned patients admitted to the Caumatology and Reconstructive Surgery service of the Vladimir Ilich Lenin University Hospital, between January 2017 and December 2019. 103 patients were selected through probabilistic sampling. The semi-structured interview, observation and documentary review were used for the evaluation. Sociodemographic variables, production mode, causal agent, hospital stay and comorbidity of mental disorders. The results were processed in the statistical packages EPIDAT 3.1 and MedCalc. Results: The prevalence of comorbidity of mental disorders was low. However, the comorbidity between substance use disorders was high. Most were male, between 41-60 years old, white, with 9th grade, single, of urban origin, with children, living with other people, with accidental burns and direct fire, and a stay of 8-21 days. There was no significant association between the comorbidity of mental disorders and variables such as skin color, age, level of education, level of origin and having children. However, regarding sex, marital status and mode of production of burns, the results were significant. Conclusions: The prevalence of comorbidity of mental disorders between disorders by consumption of substances was high. An association was found between comorbidity of mental disorders and the variables sex, marital status and mode of production.
