Resumen
Introduction: The breast carcinoma which is negative to the expression of hormonal receptors (negative RH) includes the molecular subtypes Her2/neu and triple negative, that are both associated to higher biological activity and a worse forecast. Objective: To determine the real incidence of the subtypes triple negative and Her2/neu in breast carcinoma and its correlation with prognostic value clinic-pathological variables. Methods: A descriptive, cross-sectional study was done in the Hospital Celestino Hernández, Villa Clara, from January 2017 to June 2019. It was included 293 women with diagnosis of infiltrating breast carcinoma, whose biopsies were studied by immunohistochemistry, determining the incidence of the molecular subtypes triple negative and overexpression of Her-2 and its correlation with others prognostic value variables. Results: In the series it was determined the incidence of the molecular subtypes triple negative and Her2/neu. In both molecular subtypes more two third of patients were older than 50 years old, had tumor size larger than 2 cm at the moment of diagnosis and had ductal histology. The correlation of the both molecular subtypes with moderately and poorly histological types of breast carcinoma stands out. In the same way the proliferation index determined by Ki67 was high in more two third of the studied patients. Conclusions: The post-menopausal age, the ductal histologic type, high histologic grade, high index of Ki67 and tumor size larger than 2 cm are often associated to molecular subtypes of breast carcinomas which are negative to the expression of hormonal receptors. Resumen
Introduction: Cancer constitutes one of the fatal first causes in the developed world, it includes multiple factors and these malignant diseases occupy the large-number attention of worldwide investigators. Objective: To identify the scientific available evidence on the stomatological attention to the patient with oral cancer. Methods: A review of the literature was carried out, and a combination of terms and Boolean operators were used in search engines such as The electronic databases PubMed, Medline, Cochrane, Hinari, and SciELO. For the information search the keywords used were dentistry attention, oral cancer, need for dentistry treatment, head and neck cancer, treatment of oral cancer, and radiation therapy. Results: It is transcendental that the general or specialist dentists have comprehensive training in the handling of the patient with cancer, to identify previous to the start of the oncological treatment, those clinical stages of risk that can complicate to future, using a clinical and radiographic exhaustive evaluation, besides, using constants inter-consultations with the attending physician, to point the way to the type of treatment to choose, as to accomplish it and the moment of bigger certainty to proceed with the same, as well as, the limitations and complications that can come from accomplishing or not said treatment. Conclusions: Oral cancer at present is the center of attention at the international level, a problem that hardly exonerates someplace on the planet; its incidence is appalling. The scientific-technological advances in their treatment are the possible solution for this disease with a valuable percentage of survival, but the side effects that they generate cannot be discarded during and after the treatments, for the ones that dentists should be ready to embrace measures of prevention, diagnosis, therapeutics, and follow-up of these complications. Resumen
Introduction: Bezoar is defined as the accumulation of undigested foreign material found in the digestive tract of people and some animals. They are generally located in the stomach. Medical and endoscopic treatment is the one of choice, although it will also be determined by its location and its cause. Surgical treatment is reserved when there is any complication or endoscopic therapy fails. Objective: To present a clinical case of a patient with complete pyloric obstruction due to phytobezoar and anthropyloric tumor. Case report: A 54-year-old female patient with a history of abdominal pain in the epigastrium, loss of appetite, asthenia and dyspeptic disorders of nine months of evolution. Imaging studies reported the presence of foreign bodies in the stomach corresponding to bezoars. Transoperatively, a tumor is also evidenced in the anthropyloric region. Bezoar extraction and subtotal gastrectomy were performed. Conclusions: The surgical conduct in case of gastric phytobezoar will be determined by the failure or contraindication of medical and endoscopic treatment. Resumen
Introduction: Radiobiology is the science belonging to the biology that studies the effects of the different types of radiation on the lively matter. Objective: Identifying the scientific evidence in relation to radiation therapy of the patient with cancer of the maxillofacial region. Methods: A literature revision was carried out in search engines as: The electronic data bases PubMed, Medline, Cochrane, Hinari, and SciELO. For the search for information the key words used were head and neck cancer, treatment of the cancer of the maxillofacial region, radiation therapy, brachytherapy and types of radiation therapy. Results: For the treatment it is needed to evaluate the characteristics of the tumor and to determine the tumor stage, the location and extension of disease, as well as the pathological findings that dictate the conduct made suitable like the surgery, radiation therapy, chemotherapy, hormonal therapy, immunotherapy and directive therapy. The maximum dose tolerated relating to the critical organs oscillates between 50 and 76 Gy. With the brachytherapy the radioactive source inside the patient can place itself in reservoirs, close to the tumor or in the tumor area. This allows managing dose lifted of irradiation very effective, to the tumor or zone of risk. The adverse effects of the ionizing radiations according to the impaired cell can be genetic and somatic damage. Conclusions: In the treatment of cancer, radiation therapy is a primary option, always with the precept that offers good results as to efficacy, tolerance and quality of life, causing the smaller possible damage to the sick and with the new technologies it can increase success and guarantee that a procedure keeps on being high-quality.
