This article examines petitions for name changes submitted on behalf of children and adolescents in cases where the biological father is voluntarily absent and has been effectively replaced, in terms of paternal functions, by the mother’s new partner. The cases under consideration require a nuanced reflection on the relationship between one’s name and the right to identity. The study also addresses the issue of legal representation of the child whose surname is being challenged, as well as the impact of evolving family dynamics—particularly the emergence of reconstituted families. Finally, the analysis connects name change petitions to the principle of progressive autonomy and the child's right to be heard.

Background: Obesity is important in terms of the psychosocial burden it creates, and bariatric surgery has demonstrated be an effective treatment.Objective: To perform a qualitative study in order to evaluate stigmatization and psychosocial status of individuals who underwent bariatric surgery.Material and Methods: People who had undergone bariatric surgery were interviewed online. Data were collected using a semi-structured interview form. Participants were included voluntarily. The interviews were transcribed and analyzed with the Atlas.ti. Main themes and sub-themes were revealed.Results: Ten participants were included. Mean age was 39.2 years, and 7 of them were female. The presence of unhealthy diet, inactivity, psychological status, and genetic predisposition were recorded.Most respondents felt that society is cynical when it comes to its opinion of obese people. Obesity had a negative impact on their work performance, relationships and exposed them to stigmatization. Mostof the participants had experienced a lack of self-confidence before surgery and consulted a dietician to lose weight but became stressed and regained the weight lost. The participants decided to undergo bariatric surgery and 6 of them reached their target weight while 4 of them could not. The participants stated that bariatric surgery had positive effects on their health. Half of the participants stated their social lives improved significantly after bariatric surgery and that stigmatization decreased. Most of them stated that society should be educated about its attitude towards people with differences.Conclusion: Obesity induces stigmatization and negatively affects various aspects of individuals, that improved after bariatric surgery.

Background: Background: Critical view of safety and systematic intraoperative cholangiography are the most accepted strategies to prevent bile duct injury (BDI) during laparoscopic cholecystectomy.Objective: The aim of this study was to describe the incidence, severity and management of these injuries during laparoscopic cholecystectomy that occurred despite the use of both strategies.Material and methods: We conducted a retrospective, descriptive and observational study of planned laparoscopic cholecystectomies performed between July 2008 and July 2018. Demographic data, preoperative diagnoses and intraoperative findings were analyzed.Results: During the period analyzed, 2447 planned laparoscopic cholecystectomies were performed. The critical view of safety was obtained in 2425 (99.1%) patients and 2437 (99.6%) patients underwent intraoperative cholangiography. Five patients (0.2%) a preoperative diagnosis associated with difficult cholecystectomy, developed BDI, 4 of them involving aberrant right bile ducts. All the cases were diagnosed and managed during the original surgery.Conclusion: Bile duct injuries, particularly those involving aberrant right bile ducts, can occur in patients with diagnoses associated with difficult cholecystectomies despite the use of the critical view of safety and intraoperative cholangiography.

Background: End-stage achalasia is characterized by severe dilation and tortuosity of the esophagus and the clinical presentation includes dysphagia and progressive malnutrition.Objective: The aim of this presentation is to present a case series of megaesophagus due to end-stage achalasia that was managed with surgery.Material and methods: We conducted an observational, retrospective and descriptive study. All the patients with end-stage achalasia operated on using a minimally invasive approach between March 2008 and October 2023 were included in the study.Results: In the study period, 8 patients (mean age 56 ±11.2 years; 5 were female) were treated with minimally invasive techniques for end-stage megaesophagus due to achalasia. One case corresponded to megaesophagus due to Chagas disease and the other 7 were due to recurrence of symptoms after a Heller-Dor myotomy that had been managed with endoscopic dilatations. The following techniques were used: 5 cardioplasty procedures, 1 laparoscopic transhiatal esophagectomy, 1 minimally invasive esophagectomy in prone position and 1 resection of the esophagogastric junction and Roux-en-Y bypass.Conclusion: In the series presented here, minimally invasive cardioplasty was the most used technique for the treatment of end-stage achalasia, with a low percentage of complications. Other techniques were also used for other selected cases based on a multidisciplinary approach.

The adrenal gland is a common site of metastasis but isolated adrenal gland metastases are rare. The true incidence of adrenal involvement secondary to cutaneous melanoma is unknown, but the literature describes autopsy series with an incidence of 36-54%. Currently, the decision-making and management of melanoma metastases takes into account factors such as location of metasis, disease burden, need for adjuvant therapies, and patient’s clinical status. We report the case of a 60-year-old male patient with a diagnosis of melanoma in the right subscapular region with a Breslow thickness of 10 mm and lymph node metastases who underwent tumor resection and axillary lymph node clearance. He received postoperative treatment with pembrolizumab. One year after the initial diagnosis he was diagnosed with an isolated adrenal gland metastasis. The patient underwent laparoscopic adrenalectomy and continued with systemic therapy.

Stump appendicitis is a rare complication of appendectomy with an incidence of 1 in 50,000 cases. The most common cause is failure to identify the appendiceal base in severe inflammatory processes. Although the clinical manifestations may be the same as those of appendicitis, the surgical history delays the diagnosis, which is often made during surgical reintervention because imaging tests such as ultrasound (sensitivity of 44% and specificity of 93%) and computed tomography (sensitivity of 97% and specificity of 94%) may overlook it. The diagnosis is made with a mean delay of 2 days and may be associated with complications as perforation.Although the figures vary, between 55 and 65% of the cases had a history of open appendectomy while the history of laparoscopic appendectomy was reported in only 38% of the cases. Stump length greater than 5 mm seems to be a determining factor in the development of stump appendicitis and the recommended treatment is complete appendectomy. The aim of this review was to identify the surgical history of the included cases, the time interval between the initial and second episode, the clinical presentation, the diagnosis and the treatment implemented to identify possible risk factors and establish preventive measures

Chylous ascites or chyloperitoneum is the extravasation of milky chyle rich in triglycerides (> 200 mg/ dL) into the peritoneal cavity. The etiology includes congenital anomalies, neoplastic obstructions and trauma in abdominal lymph vessels; yet, 50% of the cases are idiopathic.We report the case of a 40-year-old woman with sudden, acute back pain and no other signs or symptoms who evolved with abdominal pain and distension. The computed tomography scan detected free intra-abdominal and retroperitoneal fluid. An exploratory video-assisted laparoscopy revealed free intra-abdominal fluid with a cloudy milky-white appearance. The peritoneal cavity and retroperitoneal space were washed and a drain was placed.Peritoneal fluid analysis confirmed the diagnosis of idiopathic chylous ascites. Treatment with highprotein, low-fat enteral feeding supplemented with medium-chain triglycerides was initiated, with favorable outcome. The laparoscopic approach allowed for confirmation of the diagnosis and exclusion of associated lesions.

Ectopic thyroid tissue is characterized by the presence of thyroid tissue in locations other than its usual pretracheal position. We report the case of a 79-year-old asymptomatic male patient who presented with a right lateral neck tumor which had developed within the past four months. The imaging tests were suggestive of thyroid carcinoma with cervical lymph node metastasis. The patient underwent total thyroidectomy and R2 resection of a lateral tumor mass with invasion of deep planes and the primitive carotid artery. The pathology examination of the thyroid gland reported absence of neoplastic cells, and a poorly differentiated papillary thyroid carcinoma with anaplastic focus was observed in the lateral neck tumor. Thyroid carcinoma arising in ectopic thyroid tissue is an extremely rare entity, and a poorly differentiated papillary carcinoma with anaplastic foci is even more uncommon.

Benign multicystic peritoneal mesothelioma (BMPM) is a rare peritoneal tumor characterized by multiple cystic lesions in the peritoneal and pelvic cavity. It mainly affects women of childbearing age and is associated with risk factors as endometriosis, pelvic inflammatory disease and prior abdominal surgery.We report two clinical cases of women of childbearing age with intraoperative diagnosis of BMPM, with a description of their history, type of treatment chosen and postoperative follow-up.
