Traumatisms, wounds and contusions of liver may give rise to holes in arterial vessels of varying importance, of branches of the portal vein and of biliaryduet within the hepatic parenchima. Under such circumstances traumatic arteriovenous fistulas may occur and in turn, lead to portal hypertension ( and ascitis in sone cases), hemobilia and hepatic lesions (ischemic, infections or obstruction of biliary ducts), hese may explain hepatic cholics and deterioration of general condition of patients, leading to the patient's death as consequence of these disorders or of hemobilia. Selective arteriography permits diagnosis of this condition which is Hable to occur in any import¡nt !ver traumatism. .Surgery is employed to interrupt communication of this fistula, often through ligature of corresponding arterial or venous vessels and resectionof the hepatic territory tributary to such vessels; less frequently through suture of communication preserving vascular integrity.

The different techniques of biliogastric anastomosis: common hepaticogastrostomy, Hepaticocolecystogastostomy and Hepaticogastostomy of Gohrbandt and Dogliotti, have been analized. All of them are not standing nowadays. Seven observations of biliogastric anastomosis with various techniques are summarily reported. Among them three (transgastric hepatostomies) with transhepatic "U" tubes are poi nted out. All of the treated cases were advanced biliary cancer, with the exception of sclerosing colangitis.With a simple and quick operation, this procedure allows the drainage of the bile from the hepatic .to the stomach, leaving a changeable tube passed through between the liver and the stomach without need of suturing one organ with the other. It is used in patients in serious conditions with limited overliving, instead of doing left hepatico-jejunostomies,that are more laborious operations for this kind of patients.

Reconstructive surgery of acute arterial m¡uries had been carried out with succes for the past 15 years. With venous injuries however, ligation rather thanrepair of the. veins continues. Ligation favorises massive distal venous thrombosis This venous thrombosis is much more responsable of complications, like acute obstruction of a concomitant repaired artery or an artery, which suffered minimal lesions, which by themselves would not have determined no failure, than simple hgation. Vascular injuries need an aggressive approach to repair both vessels. lf only there is carried out the arterial repair, with ligation of the corresponding vein, the defered venous repair must be made, specially when extense venous thrombosis is present. On hand of 28 vascular injuries by gunshot of extremities, authors systematize therapetic measures.

This report covers a series of 14 patients who underwent substitution of mitral valve. In 11 cases StarrEdward's valve was employed and in the remaining three the valves were made of dura mater. Reasons for surgery, tactics and techniques employed and postoperatory results are described.

The author's experience with choledochoduodenostomyes is based on a series of 28 patients. He startsby a chronological outline of this procedure. It is a valuable too! in the, treatment of multiple residual and/or recurrent choledocholithiasis, cholangitis and subacute or associated chronic pancreatitis, and more specially so in the case of elderly patients, in reoperations of the biliary t ract and in those cases where risk is considerable. Technical details and objections to its application are listed. The author concludes that is an advisable procedure. It does create a pathological condition, but it helps to eliminate biliary disease which is worse. Results obtained ·have been excellent.

After childbirth a patient underwent surgery for choleperitoneum due to permeation. During pregnancy she had suffered from obstructive jaundice. Surgerywas performed in an acute phase, 15 litres of biliary liquid were drained from a preperitoneal pouch but the biliary tract, which was not drained, was not perforated.A fortnight later cholelithiasis was suspected, she was re-oper-ated _and a gallstone imbeded in the papilla was removed; this was the determinant cause of both jaundice and choleperitoneum and had possibly been accompanied by acute pancreatitis and backflow caused by Opie's mechanism.

One case of duodenal adenomatous Brünnerian polyp whose only symtomatology was pain in the right upper abdomen is reportea. The operative diagnosis was tumor in duodenalbulb. Gastropiloroduodenostomy was made with a large duodenal resection and gastro duodenal anastomose. The definitive diagnosis of the lesion was histologic.The highest degree of rarity about this sort of tumor is pointed out.

In surgical treatment of renal cancer in both adults and children, the thoraco-abdominal approach is advisable.In the child the need far urgent diagnosis is greater and consequently approach may only be abdominal; in such cases it is useful to remember that Wilms' tumor is highly friable. The author winds up his report by stating which are, in his opinion, those conditions that render resection of such tumors impossible.
