Resumen
Surgical anatomy and operatory tactics in danger area in thyroid surgery recurrent nerve, inferior thyroid artery, parathyroid gland are analyzed. Recurrent nerve: Obligatory visual and palpatory identification helps reduce the number of permanent ¡'aralyses of vocal chords and makes possible safe hemostasis and gland resection. The danger of nervous lesions -regardless of quality of surgical technique employed-, is real in anatomical ( non recurrent inferior laryngeal nerve) and pathological (multinodular, endothoracic goiters) situations which distort normal path of nerve, displacing it forward. Inferior tl,yroid artery: Interruption of arterial flow by simple ligation, without section, at a distancef

Resumen
The author describes approach to cervical goiter. and analyzes all inherent aspects starting by position of patient on operating table and up to type of anesthesiaemployed. He then considers the different incisions possible, as well as the different manners of cutting the cutaneous flap, and finishes with anatomical and functionalconsiderations on useful surgical space and infrahyoid muscles.

Resumen
The author comments on anatomical characteristics of superior pole of thyroid gland and its incidence onsurgical technique during the course of thyroidectomies. Surgical steps in the management of the superior pole of thyroid are analyzed. They are: exposure, disinhibition, hemostasis of superior pedicle and descent of pole. Adequate manouvres for each step are considered, with stress on anatomical knowledge and its application during surgery. Risk involved in such manouvres is mentioned; and exhaustively analyzed; possibility of haemorrhage, nerve aggression, visceral lesions and aggression of parathyroid glands.

Resumen
1) Dextran deriva tes are excelent plasma expansors. Consequently they are indicated in conditions the prevention and treatment of hipovolemic.2) They have two other biological effects which have clear indications: a) Improvement of microcirculation, Dextratrane 40.000. b) Antithrombotic effect, Dextrane 70.000.3) Due to their action mechanism, hydroelectrolitic infusions should be infused at the same time in order to avoid depletion of the interstitial space. 4) The usual dose is 500 ml. It is not convenient to go beyond 1.000 ml.

Resumen
The mora outstanding properties of hydrosaline solutions are considered, with stress on the fact that they facilitate miocardial function as a mechanic pump. Molar solutions, their management and reduced water volume employed are mentioned, Hydrosaline solutions should cover water and electrolitic requirements, previous deficits, anticipated losses and additional pathological losses. With respect to the manner of administering them, wme. should be administered early and rapidly, while others require diferred and slow methods.
