Resumen
Introduction: Cardiovascular dysfunction is the first cause of death in patients with chronic kidney disease.
Methods: Observational, longitudinal and prospective study, which included 58 patients with functioning renal transplant who were studied different echocardiographic variables, before and six months after the closure of the arteriovenous fistula.
Results: The average age was 46.62 years and the most frequent was male 30 (51.72%). The flow of angioaccesses at the wrist level was demonstrated in 28 patients (45.9%) with evidence of regression of mean pulmonary artery pressure from 25.56 to 19.58 mmHg (p=0.002), more connoted for those of intermediate flow; 14. Also, the area of tricuspid insufficiency was greater for that group; from 3.15 to 1.96 cm2 (p=0.001). The pattern of mild dysfunction was observed in 38 patients (62.3%) prior to closure. Subsequent to this 27; (51.9%), exhibited minimal valvular insufficiency and 9 individuals (14.9%) did not show any degree of valvular dysfunction.
Conclusions: Closure of the arteriovenous fistula in patients with functioning renal transplantation contributed to decrease the risk of pulmonary hypertension and progression of tricuspid valve dysfunction.

Resumen
The occurrence of complications affects the evolution and darkens the prognosis of patients with spontaneous subarachnoid hemorrhage. The objective was to determine the incidence of neurological and non-neurological complications in a series of patients with spontaneous subarachnoid hemorrhage. An observational, descriptive-correlational and retrospective study of 96 patients with spontaneous subarachnoid hemorrhage between 2016 and 2021 was carried out. The frequency of different neurological and non-neurological complications was determined, as well as the relationship between the presence of neurological complications and other variables through analysis. bivariate (Odds Ratio and its confidence interval). Among the results it is found that 53% of the patients presented some type of neurological complication, which prevailed in those 60 years of age or older (62% vs 45%), in aneurysmal etiology (54% vs 27%) and when the diagnosis was made. in the first 48 hours of the onset of symptoms (56% vs 43%). The frequency of complicated patients increased from grade I to grade V on the Hunt and Hess scale (OR 30.6[8.2;113]), the same as on the Fisher scale (OR 11.6[3 ,1;43,2]). The most frequent neurological complication was intracranial hypertension (39%), while the non-neurological one was pneumonia (24%). It is concluded that the series of patients was characterized by a high frequency of neurological complications, among which intracranial hypertension predominated. An evident relationship was demonstrated between the worst grades on the Hunt and Hess and Fisher imaging scales, obtained in the patient's assessment on admission, and the higher incidence of neurological complications.

Resumen
Kyphoplasty is one of the minimally invasive interventions which provides pain relief in osteoporotic or malignancy related vertebral compression fractures. Severe complications associated with percutaneous kyphoplasty or vertebroplasty are rare, and usually related to cement leakage out of the vertebral body (Lamy et al., 2014). We report the case of an 83-year old female who suffered an ischemic stroke during the immediate post-operative period after a three-level percutaneous kyphoplasty (L2-L3-L4) due to a paradoxical embolism via an atrial septal heart defect.

Resumen
Chronic rhinosinusitis (CRS) is a chronic condition of inflammation in the mucosa of the paranasal sinuses that is associated with a significantly impaired quality of life, which motivates the constant study of therapies for its control and cost reduction. The cutting edge of clinical management for CRS is topical therapy. The advantages of topical medical therapy include direct administration of the drug into the diseased tissue, the possibility of administering higher local concentrations of the drug and minimizing systemic absorption which is not without adverse effects, thus becoming an important strategy in the management of CRS. The objective of this literature review is to formally review and identify topical medical therapy strategies and promote an evidence-based approach to their use. A literature review was conducted based on a keyword search. The computerized databases of PubMed, ElSevier and ResearchGate were accessed. We included articles highlighting the results of the use of topical therapies in CRS and that the type of study was a clinical trial. This review identified and evaluated the literature on topical therapy strategies for CRS: saline irrigation, topical antibiotics, topical steroids, and topical antifungals. Topical therapy has become an important tool in the treatment arsenal for CRS. Based on a review of the literature, nasal irrigation with saline and topical steroid sprays are the most evidence supportive. Nonstandard topical nasosinusal steroid therapies may be an option for managing CRS.

Resumen
This editorial reflects on the emergence of artificial intelligence and how it is applied in the field of health. AI is a technology that mimics the functioning of the human brain and uses algorithms, machine learning, pattern recognition and cognitive computing to process large amounts of data and extract useful insights. AI can improve health and well-being of people in various areas of medicine, such as medical image analysis, diagnosis, treatment, genetics, pregnancy and smart prosthetics. These applications can help medical professionals make better clinical decisions, accelerate research and development of new drugs, personalize patient care and reduce costs and human errors. AI can also empower users to access relevant information about their health and actively participate in their care. However, AI also poses ethical, legal and social challenges that must be addressed with responsibility and transparency.

Resumen
Objective. This article presents the results of a qualitative research. Its objective was to analyze interculturality in virtual education in a public educational center in the city of Ayacucho. Methods. The methodology used was classroom observation and semi-structured interviews with teachers, students and parents. The information obtained was subjected to a process of data reduction and analysis. Results. The main results indicate the limitations in the accessand use of information and communication technologies by the teacher and students, as well as the adaptation of the content of the classes to the local reality by the teacher. Conclusions. These conditions have resulted in a greater "rapprochement" of the students with the people of their environment and the culture of their origin, which, instead of contributing to cultural dialogue and intercultural education, has rather fostered endoculturation.

Resumen
This article reflects on the interculturality of the human right to peace in Mexico to recognize how the encounter between cultures is fostered where economic, cultural and social elements are exchanged, highlighting that conflicts can arise and derive in violence transgressing peace. The proposal of real law is described, which uses the intercultural method to consider the customary law of native peoples, their traditional principles, as well as their cosmovisions and build peaceful environments. Challenges, proposals and strategies involving native peoples were also identified to guarantee the right in question. Finally, it is invited to consider the legal, theoretical and practical elements presented to continue developing empirical approaches from the intercultural approach for peace that will allow the formation of critical and conscious subjects of cultural differences as an area of opportunity to guarantee the human right to peace.

Resumen
La presente investigación es la continuación del análisis legal del delito de piratería en altamar en todo el mundo, ahora focalizado en los ataques piráticos en el mar Adriático, Mediterráneo y el Egeo; pero sin abarcar España y Francia, ni los países africanos debido a ya haber sido abordados en una investigación diversa, y que ello implicaría una extensión impermisible al presente. Contextualizado lo anterior, primeramente se citarán los casos más notables históricamente, que al caso fueron en lo que ahora es Grecia, Roma y Turquía; posteriormente se abordará el estudio de los códigos penales de todos los países que conformar la zona ya referida, para exponer la premisas de sus respectivos tipos penales, así como señalar cuales naciones no tienen tipificado nominalmente este delito y si de ellos se puede verificar algún tipo penal que, por sus premisas acotadas, lo tenga regulado bajo otra nomenclatura. Debe hacerse mención que las referencias del tema son tan amplías como diversas en su idioma respectivo, ya que no son del habla hispana; no obstante, fueron seleccionadas aquellas consideradas como más eficaces.

Resumen
El artículo se propone analizar el Estado de Derecho en México y el incremento de la corrupción en el periodo que va de 2015 a 2021, así como la herramienta creada para su combate: el Sistema Nacional Anticorrupción (SNA). Para ello, se construye un marco teórico y conceptual partiendo del postpositivismo jurídico y la postura liberal igualitaria, con ellos se puede percibir que el SNA no ha funcionado como herramienta eficaz en el combate a la corrupción y el fortalecimiento del Estado de Derecho. Considerando normas y estudios sobre la materia, se realiza el análisis del SNA desde su estructura, función y fin, identificándose problemas de coherencia y un mínimo impacto en la realidad, hallazgos que hacen evidente la necesidad de fortalecer la institucionalidad del SNA y generar consensos democráticos sobre la forma de combatir la corrupción.

Resumen
Objective. To analyze, based on the judgment of experts in the feld, Peruvian labor regulations in the public sector during the last five decades, comprising thirteen governments between 1968 and 2021, from the presidency of General E. P. Juan Velasco Alvarado to that of Engineer Francisco Rafael Sagasti Hochhausler. Methods. The researchdeveloped was of a basic type, employing a quantitative approach that produced a descriptive level study, under a nonexperimental research design of transversal cut. The sample was obtained through a purposive sampling procedure, and consisted of 10 doctoral students enrolled in the Thesis Seminar III of the Doctorate in Law of the Graduate School of the University of Huánuco (Peru), to whom a survey was applied, administered through a questionnaire. Results. There is a vast and differentiated labor regulation in the Peruvian public sector, where there are 15 different labor regimes, in a context of unresolved structural labor problems by the current legislation, which contributes to increase labor instability in the public sector, whose responsibility lies with the national, regional and district governments; all of which reafrms the legal meaning of labor stability as a contractual link between the State and the public employee. Conclusions. The Peruvian public labor regulatory policy of the past five decades is in general terms coexistent, dispersed and unequal, and has maintained the incoherent effects on the welfare of the public worker based on the coexistence of the private labor regime within the public sector.
