ARK: http://id.caicyt.gov.ar/ark:/s25458299/wqz4qxqrf

Introduction: Pregnancy is a physiological state of several anatomical, hormonal, hemodynamic andelectrophysiological changes. Wolff-Parkinson-White syndrome is a congenital abnormality in the electricalconduction system of the heart, with suggestive changes in pre-excitation in the electrocardiogram, WHOtargets in reducing neonatal maternal morbidity. Objective: Describe the clinical case of a pregnant patient with cardiac pathology compatible with WolffParkinson White Syndrome in treatment with beta blocker drug. Material and methods: Retrospective descriptive study, clinical case presentation. Results: 17-year-old female sex patient with a history of Wolff Parkinson White Syndrome (WPW) diagnosed2 years ago 7 months ago in treatment with Carvedilol 6.25mg once daily with regular monitoring withcardiologist. Attends subsequent pregnancy control (24 weeks gestation) with laboratory test result withinnormal parameters, refers to palpitations, accompanied by sweating, asthenia and vertigo of an hour ofevolution for which 6.25 mg of additional carvedilol at the usual dose without improvement. At the timeScore MAMA 3 at the expense of the heart rate (200-220 lpm) so it is transferred with all primary measures tounit of greater resolution capacity for stabilization and evaluation by cardiologist, which suspends usualmedication and starts Atenolol 50 mg once daily until end of pregnancy, explains possible fetal side effectsand continues with prenatal and pathology-based checks with cardiology. Conclusions: Scientific evidence on beta blockers and safety during pregnancy remains a source of study andcontroversy, so new research will be needed in the coming years to further clarify this aspect.

Introduction: Megacystis microcolon intestinal hypoperistalsis syndrome (MMIHS) or Berdon syndrome is a severe autosomal recessive congenital disorder characterized by functional bowel obstruction, bladder disten-sion or urinary retention and intestinal hypoperistalsis from birth in addition to other alterations. It has a very low incidence and life expectancy barely exceeds a year with a poor quality and usually dying due to septic processes. For his diagnosis the Radiologist plays a fundamental role, since the radiological tests are mostly pathognomonic. In the management of these patients, nutritional support is fundamental, which is often based on parenteral nutrition for long periods of time. In spite of the improvement in the assistance to these patients, this syndrome continues presenting a high morbidity and mortality being the intestinal transplant a therapeutic alternative. Objective: To describe a clinical case of Berdon syndrome or Megacystis microcolon intestinal hypoperistal-sis syndrome, presented in EcuadorMaterial and methods: Retrospective descriptive study, clinical case presentation Results: The following is a clinical case of a female patient who presented poor oral tolerance, bloating, and low urinary output from birth; radiological and clinically she was diagnosed with Syndrome and Berdon, she received intrahospital parenteral nutrition without good weight gain, poor quality of life and she dies at two months due to sepsis. Conclusions: Berdon syndrome is a rare disease with low incidence that presents high morbidity and mortality despite the advances in its management

Introduction: The rejection of a blood transfusion and blood products is a specific situation of the patient'sright to any treatment and is based on the principle of autonomy exercised through consent. This right istested when vital treatment is rejected, which can cause contraatity and doubts in the professional'sperformance. This article reviews ethical principles, religious principles, and medical-legal foundations of the patient and act with legal certainty. Objective: Identify the biblical foundations of Jehovah's Witnesses for rejection of blood transfusions.Analyze from a bioethical perspective the conflict of hamotransfusing a patient Jehovah's Witness and itssolution. Material and methods: A systematic review of the literature and documents of scientific societies devoted tothe study of the controversy over the applicability of the ethics of blood transfusions in Jehovah's Witnesses,as well as systematic reviews, meta-analysis and scientific studies over the past five years. Search strategy:Electronic searches were conducted on MEDLINE, EMBASE, LILACS located through PubMed. Results: The initial search founded 52 studies on the subject, although 32 were excluded that were notrelevant to the purpose of this review. Finally, 12 systematic reviews, 1 original study, 4 meta-analysis, and 3bibliographic reviews were selected. Conclusions: Training doctors in blood-saving methods and a better understanding of the religiousfoundations of the problem, as well as the ethical aspects and legal framework in our country, can helpimprove communication between the doctor and the patient Witness Jehovah, preventing conflictingsituations that in some cases cause a delay in treatment that can negatively impact the patient's health.

Introduction The new coronavirus epidemic (SARS-CoV-2), first reported in December 2019 in Wuhan, China, and which has been declared a public health emergency of international concern by the World Health Organization, shows a substantial morbidity and mortality, for this reason it is essential to deepen the epidemiological, clinical aspects of diagnosis and treatment to face and overcome this scourge. Objective: Review the best published medical evidence and analyze the behavior of the SARS-Cov-2 pandemic, its epidemiology, isolation, clinical manifestations, complications, the most significant elements for its diagnosis and the new options to learn and apply these experiences in new epidemics. Material and methods: A systematic search was performed on the PubMed, ProQuest, Embase, Redalyc, Ovid, Medline, DynaMed and ClinicalKey, EMBASE, LILACS databases during the period 2019-2020 in the international, regional and local context. Results: The systematic review of articles provided a total of 100 records, of which 62 were eligible; of these, 12 were not relevant to the subject matter of this review, so 50 were included. Conclusions: The disease caused by SARS-Cov-2, despite its high mortality, can be prevented with isolation, diagnosed in a timely manner and effectively treated, which will reduce its epidemiological impact worldwide. There is a potential therapeutic arsenal in the experimentation phase with promising results

Introduction:The unique ventricle is a cylonotizing congenital heart disease with an estimate of 0.05 to 0.1 cases per10,000 birth. It has a unique ventricular chamber where atrioventricular valves drain blood from the cave andpulmonary veins. It is accompanied by other abnormalities, the transposition of large vessels is morecommon. Early surgery improves life expectancy. Objective:Describe a clinical case of a patient with a single ventricle. Material and methods: Descriptive, retrospective study, clinical case presentation of single ventricle. Results: It describes the case of a 9-year-old patient with single ventricle plus large vessel transpositiondiagnosed at 2 months of age treated with drugs for refusing to surgical intervention. Conclusions:The unique ventricle is a rare congenital heart disease that requires a complete physical examination at birthaccompanied by imaging tests for the purpose of early diagnosis and intervention.
