Life satisfaction is the psychological state that arises because of the individual’s interaction with his or her environment. Life satisfaction is expressed through the global assessments that people make of their lives. The goal of the present research was to analyze the psychometric properties of the Satisfaction with Life Scale in older adults. The sample included 605 participants, from the state of Chihuahua, Mexico. The structure of the questionnaire was examined using confirmatory factor analyses. The results show evidence that a single-factor structure is viable and appropriate, as it shows adequate fit, reliability and validity indicators. The Satisfaction with Life Scale is a useful tool for the study of the factors that determine subjective well-being in populations of older adults. However, it is necessary that future research delve into its use, this will allow for more robust evidence in this regard.

The current predominance of finance and an economy of debt has been analyzed by Maurizio Lazzarato as having negative subjective effects on the decision-making capacity, the perception and value of time and on the effects of debtors. Guilt is highlighted as the most relevant and dangerous affect, both personally and politically. To this sociological perspective, psychoanalysis contributes a perspective linked to the subject, his desire and his enjoyment, mediated by the structure of the unconscious. The proposal is that the concept of superego created by Sigmund Freud and elaborated by Jacques Lacan, allows a better understanding of the mechanisms and devices that produce both guilt and debt in subjects. The analysis of the superego reveals the inverse relationship between desire and guilt and how the latter is the product of an immune strategy of rejection of castration.

Introduction and objectives: seventy five percent of bladder cancer is non muscle invasive (NMIBC) for which intravesical instillation with BCG has been the cornerstone of treatment. BCG has been categorized as dangerous; this is why there are international guidelines that describe its use. The objective of this paper is to investigate the adherence of Latin American health workers with guidelines for the safe intravesical application of BCG for NMIBC.   Materials and methods: we conducted an anonymous and self-administered survey during 2 months of 2023 on a digital platform among health workers. It consisted of 19 questions focusing on compliance with standards set by international organizations and its correct use. Descriptive statistics were carried out; p<0.05 was considered statistically significant. Correct and safe use of BCG was evaluated through 4 questions.   Results: a total of 295 completed surveys were recorded from 12 countries. A total of 96.61 % (n= 285) performed BCG instillation incorrectly. Prior training was received by 55 % of which 97.5 % administered it incorrectly against 95.48 % who did so without prior training (p=0.33). Before answering this questionnaire, 90.51 % considered their technique adequate against 83.83 % after answering it, observing a change in the perception of participants (p= 0.0139).   Conclusion: there is almost no adherence of health professionals with protocols, so it is crucial to improve training based on international guidelines to minimize the risk of BCG contamination.

Clinical case description: the case of an 84-year-old male is presented, who developed penile glans necrosis over several days following prostatic arterial embolization. Due to the progression of the condition, a partial penectomy had to be performed for resolution. Relevance: prostatic arterial embolization represents an effective approach for benign prostatic hyperplasia, although it is not without risks. As embolization of non-target areas is a very unusual but serious complication, there is a need of knowing and describing this kind of results. Clinical implications: despite the unusual nature of penile glans necrosis, early recognition and diagnosis is crucial to prevent the worse outcomes. There are several clinical treatments before a surgical option that includes hyperbaric oxygen therapy or phosphodiesterase 5 inhibitors wich have a short opportunity window. Conclusion: gland necrosis is a rare but serious condition that requires intensive and constant management to try to achieve its reversal. Ultimately, its treatment consists of partial or total penectomy.

Clinical case: a 20-year-old male patient with type 1 obesity presents with abdominal and dorsal pain. Pathological diagnosis reported Wilms tumor (WT) stage IV. He received preoperative chemotherapy and subsequently underwent left radical nephrectomy. Currently, the patient remains asymptomatic and without evidence of recurrence 12 months after surgery. Relevance: Wilms tumor or nephroblastoma is an uncommon pathology in the adult population with very few reported cases. The advanced stage of the disease complicates surgical and oncologic treatment.  Medical implications: currently, there are no defined guidelines for the treatment of WT in the adult population due to its low frequency and the lack of information on the strategies employed in high-specialty hospitals where these cases have been managed. Conclusions: the therapeutic approach of WT in adults presents significant challenges, making it necessary to establish a standardized model of care and management. The partial response to chemotherapy and the appearance of new lesions indicates the possibility of requiring additional treatments, as well as strict surveillance. Systematic recording and analysis of these cases will contribute to the development of more effective therapeutic strategies for this rare clinical entity.

Goal: the aim of this work was to identify the various types of lower urinary tract dysfunctions (LUTD among children with Attention Deficit Hyperactivity Disorders (ADHD) and their responses to treatments.   Methods: this prospective work had been conducted on participants aged 6 -18 years old, who fulfilled criteria for ADHD with any type of LUTD and only naive patients who had no previous urological treatment.  All patients were subjected to the standard psychiatric treatment in form of Atomoxetine and/or methylphenidate. The patients were re-evaluated at 1,3,6 months to assess improvement of daytime symptoms and nocturnal enuresis (NE).   Results: in our 60 patients, there was no significant different between recurrence rate of NE and type of ADHD and NE. There was no significant different between type of ADHD according to response of enuresis treatment. Regarding response of treatment of daytime symptoms, 14 (77.7 %) patients showed full response, while 4 (22.2 %) patients did not show response. In patients with NE, 28 (66.7 %) patients showed full response, 8 (19.0 %) patients showed intermediate response, and 6 (14.3 %) patients did not show response. While in patients with non- monosymptomatic NE, 14 (77.8 %) Patients with showed full response, while 4(22.2 %) did not show response. Conclusions: a combination therapy of desmopressin and imipramine is efficient treatment of monosymptomatic NE in ADHD children, while anticholinergic drugs plus desmopressin are effective in non-monosymptomtic NE. Limitations: number of cases is small and further multicenter studies are needed to confirm our conclusion. Originality: this manuscript describes an original work.

Objective: To describe the state of the art on indications, complications, adverse effects, therapeutic options, and early vs. late insertion in patients with refractory priapism who underwent penile prosthesis implantation. Methods: We searched MEDLINE (OVID), EMBASE, and the Cochrane Central Register of Controlled Trials (CENTRAL) from its inception to the present. Also, accessible clinical practice guidelines from North America, Europe, South Africa, and Japan Results: The search in the various research engines found 255 articles, of which 12 were chosen after eliminating duplicates, unrelated articles, and non-human studies. Conclusion: The early use of penile prostheses is a viable option in the ischemic priapism refractory to first-line treatment, diminishing the risk of adverse events related to late implants and giving patients a possibility of preserving sexual function and, at the same time, providing therapy to an unusual pathology with significant comorbidity

Prostate cancer is a significant cause of cancer-related deaths in men. Early detection is crucial to combat the disease. RNA sequencing is a technique that has revolutionized our understanding of human transcriptome and is now a useful tool for identifying biomarkers that help in diagnosis, prognosis, and monitoring of cancer treatment. This article explores how the transcriptome has become a promising tool for guiding clinical decisions, especially in active surveillance and adjuvant treatments in prostate cancer.

Objective: To evaluate the frequency and severity of prostate symptoms in men aged 40-70 years in Tabasco, Mexico, where the lack of local data limits prevention strategies and early detection of pathologies such as prostate cancer and benign prostatic hyperplasia (BPH). Methods: Descriptive, cross-sectional study (February-August 2024) of 383 randomly selected men. The International Prostate Symptom Scale (IPSS) and a socio-demographic questionnaire were used. Data were analyzed with SPSS v25 using descriptive statistics and Spearman correlation. Results: 79.1 % of participants reported at least one lower urinary tract symptom (LUTS) in the last month, with nocturia (26.4 %) and urinary frequency (46 %) being the most common. Symptoms were mild in 54 %, moderate in 31.2 % and severe in 7 %. Age showed a positive correlation with symptom severity (0.186, p<0.001), whereas medical history (hypertension: 20.9 %, diabetes: 16.7 %) showed a negative correlation (-0.265, p<0.001). 44.9 % had no relevant medical history. Conclusions: The high prevalence of LUTS in Tabasco highlights the need for educational campaigns and free screening to reduce underdiagnosis and associated mortality. The association with age and comorbidities reinforces the importance of addressing these factors in public health strategies.
