Resumen
Objetive: To determine the most relevant associated effect of the modified NTI intraoral device for temporomandibular disorders with acute closed lock in patients attending the Dentistry service of a referral hospital during the year 2024. Material and methods: Pre-experimental, longitudinal study design. This study evaluated the use of the modified Nociceptive Trigeminal Inhibition (NTIm) device in 33 patients with acute closed lock, comprising 30 women and 3 men, distributed in age ranges between 18 and 55 years. Results: Clinical evaluations at 5, 10, and 15 days demonstrated significant improvements in mouth opening and a reduction in orofacial pain, attributable to the inhibition of trigeminal nociceptive reflexes mediated by the antero-inferior positioning of the device and complementary mandibular exercises. Conclusion: The findings support the use of this modified intraoral device as a conservative, non-invasive, and immediately applicable multimodal therapeutic strategy for the initial management of these TMDs.

La dermatitis atópica (DA) como la enfermedad inflamatoria crónica de la piel más común, con una prevalencia creciente en las últimas décadas, afectando hasta al 30% de los niños y al 10% de los adultos en países desarrollados. Su etiología es multifactorial, involucrando factores genéticos y ambientales que generan disfunción de la barrera epidérmica y respuestas inmunitarias aberrantes. Las mutaciones en filagrina, presentes en hasta el 30% de los pacientes, predisponen a una mayor pérdida de agua transepidérmica y a la colonización por Staphylococcus aureus, que exacerba la inflamación y aumenta el riesgo de infecciones secundarias. Clínicamente, se manifiesta con prurito, xerosis y lesiones eczematosas, clasificándose según la edad de inicio en formas precoz, tardía y senil. El manejo incluye emolientes, corticosteroides tópicos, inhibidores de calcineurina y terapias sistémicas como ciclosporina o dupilumab en casos graves. La DA presenta un curso clínico con remisiones y recaídas, y sus complicaciones incluyen infecciones bacterianas y la erupción variceliforme de Kaposi, lo que subraya la necesidad de un manejo interprofesional enfocado en mejorar la calidad de vida del paciente y prevenir potenciales complicaciones (1).

Resumen
Objective: To determine the risk factors associated with treatment failure of iron deficiency anemia in children under 5 years at the Pueblo Joven Señor de Luren Health Center, 2023. Methodology: Analytical, retrospective case-control study (1:1) with 110 medical records of children aged 6 months to 5 years. Treatment failure was defined as hemoglobin <11 g/dL after 6 months. Nutritional, perinatal, feeding, and maternal risk factors were evaluated. Results: Significant factors identified included: acute malnutrition (OR = 4.58; p = 0.001), prematurity (OR = 3.75; p = 0.013), low birth weight (OR = 2.41; p = 0.049), non-exclusive breastfeeding before 6 months (OR = 3.59; p = 0.002), teenage mother (OR = 6.20; p = 0.001), low maternal education (OR = 5.23; p = 0.003), and maternal anemia during pregnancy (OR = 3.95; p = 0.001). No significant associations were found with intestinal parasitosis, diarrheal disease, iron dosage, child's sex, or age. Conclusions: The main identified risk factors suggest the need to improve child nutritional status and maternal conditions to enhance treatment effectiveness.

Resumen
Loxoscelism is the pathology due to the bite of the brown recluse spider. The main enzyme involved is sphingomyelinase D and it trigger a cutaneous form, with the classic liveloid plate with subsequent evolution to a visco-hemolytic form, with systemic and eventually lethal involvement. This being the normal order of presentation of the disease, we present the case of a patient with this condition, but whose lesion did not follow the classic evolution, rapidly developing the visco-haemolytic form with renal and hepatic involvement; but with favourable evolution.
This form of presentation of loxoscelism goes out of the normal framework of the evolution of the pathology and therefore the importance of the article.

Resumen
In the most vulnerable corners of the planet, a nutritional shadow haunts pregnant women: iron deficiency anemia. This condition, defined by insufficient hemoglobin levels, knows no borders. It affects 30% of pregnant women in Sub-Saharan Africa, 25% in Southeast Asia, and 18% in Latin America. Its cruelest consequence—low birth weight—not only saps strength from newborns but also perpetuates cycles of poverty: studies in rural areas reveal that for every 100 grams lost at birth, educational potential in adolescence is reduced by 2%.

ABSTRACT
Objective: To describe the epidemiological and clinical characteristics of patients treated for moderate to severe dengue at the Regional Hospital of Ica during 2022. Materials and Methods: A cross-sectional, observational, and retrospective study was conducted. Medical records of confirmed cases of moderate and severe dengue, treated between January and December 2022, were reviewed. From a total of 578 identified cases, 232 were selected using probabilistic sampling. Data were collected using a previously validated epidemiological form. Results: A total of 45.7% of patients were between 15 and 34 years old; 57.8% were male, and 46.1% were from the central district of Ica. Most cases (82.8%) presented with dengue without warning signs, 12.1% showed warning signs, and 5.2% were classified as severe dengue. The most frequent symptoms were fever (100%), headache (85.3%), lower back pain (82.8%), retro-ocular pain (79.3%), and arthralgia (74.6%). Coinfection with COVID-19 was detected in 13.4% of patients. Virological testing revealed the presence of serotype 1 in 50.9% of cases and serotype 2 in 49.1%. Conclusions: Most moderate to severe dengue cases in Ica during 2022 affected young males from the central district. The majority presented with non-severe forms of the disease. The detection of co-circulating serotypes 1 and 2, along with COVID-19 coinfections, underscores the importance of strengthening clinical and virological surveillance in the region.

ABSTRACT
Introduction: Fibrillary glomerulonephritis (FGN) is a rare glomerular disease characterized by non-Congo red-reactive fibrillar deposits, diagnosed definitively by electron microscopy.
Case presentation: We report a 59-year-old Peruvian male presenting with progressive renal failure and nephrotic syndrome requiring hemodialysis. Renal biopsy and electron microscopy revealed fibrillary deposits in glomeruli, mesangium, and tubules. Hepatitis C virus (HCV) serology was initially positive but active infection was excluded by negative viral load. Immunosuppressive therapy with prednisone and mycophenolate mofetil was initiated.
Clinical outcome: After six months, partial renal recovery and remission of proteinuria allowed withdrawal from dialysis. At 24 months, renal function remains stable with no significant relapse.
Conclusion: FGN may show favorable response to immunosuppression in selected cases without monoclonal gammopathy or active infection, highlighting the importance of early diagnosis and sustained follow-up.

ABSTRACT
Objective: To evaluate the relationship between the degree of depression and the clinical progression of diabetic foot in patients treated at Hospital II-1 Santa María del Socorro in Ica, 2025. Methods: This was an observational, descriptive, and cross-sectional study with correlational analysis. The Zung Self Rating Depression Scale (SDS) was applied to a simple random sample of 191 patients aged 40–70 years with a clinical diagnosis of diabetic foot. Statistical analyses were performed using SPSS v25, and chi-square tests were used to assess associations between depressive symptoms and clinical variables. Results: A total of 95.8% of participants exhibited mild depressive symptoms, while 3.2% showed moderate symptoms. Significant associations were observed between the degree of depression and clinical variables, including physical activity and rest (χ² = 84.36, p < 0.05), dietary regimen adherence (χ² = 91.94, p < 0.05), and foot care practices (χ² = 85.51, p < 0.05). Conclusions: A significant relationship was found between depression and the clinical course of diabetic foot. Routine psychological screening and the inclusion of educational interventions are recommended to enhance comprehensive care for this patient population.

Background: Hospital-acquired infections (HAIs) are a frequent complication of surgical care, increasing morbidity, mortality, and healthcare costs. Although surgical drains are often useful, they may also serve as entry points for pathogens. Objective: To determine the association between the use of surgical drains and the frequency of hospital-acquired infections at Santa María del Socorro Hospital in Ica, Peru, during 2024. Methods: A cross-sectional, retrospective, and analytical study was conducted among 177 adult patients who underwent abdominal surgery. Sociodemographic, clinical, and surgical variables were analyzed using a validated data collection form. Descriptive statistics and the Chi-square test were applied to assess associations (p 7 days) was also more frequent in the drainage group (37.8 % vs. 0 %; p < 0,001). Conclusions: The use of surgical drains was significantly associated with increased frequency and severity of hospital acquired infections. Strict indications and timely removal of these devices are recommended, along with strengthened institutional prevention strategies.
