Resumen
Introduction: Sexual function during aging is conditioned by various biological, psychological, social and cultural factors. Some of the factors that may be positively or negatively influencing the sexual function of adults are sexual self-esteem and sexual dyadic communication.
Objective: To analyze the influence of sexual self-esteem and dyadic sexual communication on sexual function in adults in pre-old age and old age.
Material and methods: A quantitative correlational-predictive study was carried out, in which 247 adults in pre-old age and old age from the city of Saltillo, Coahuila, Mexico, who mentioned having sexual relations in the last 6 months, were evaluated, using non-probabilistic convenience sampling.
Results: High sexual self-esteem in adults, effective dyadic sexual communication and good sexual function were identified. Positive and significant correlations were obtained between the study variables. Likewise, it was found that sexual self-esteem and dyadic sexual communication have a positive and significant influence on sexual function in adults in pre-old age and old age.
Discussion: Sexual function in adults in the stages of pre-aging and old age depends to a certain extent on their sexual self-esteem and the dyadic sexual communication they have with their partner.
Conclusions: Sexual self-esteem and dyadic sexual communication have an important influence on the adequate development of sexual function in older adults.

Resumen
Introduction: Graves-Basedow's disease is an autoimmune entity, which in pregnancy has a low prevalence, whose clinical manifestations are secondary to thyrotoxicosis and in rare cases may have hematologic manifestations.
Objective: To describe the case of a pregnant woman with severe thrombocytopenia secondary to Graves-Basedow disease, its treatment during pregnancy and the clinical evolution of the mother-child binomial.
Case presentation: It is shown the clinical case of a 22-year-old female patient, 7 weeks pregnant, who presented vomiting up to 30 times a day, for 2 weeks, with self-limited episodes of epistaxis. She was diagnosed with thyrotoxicosis due to Graves' Basedow disease, associated with severe thrombocytopenia. Treatment included corticosteroids, antithyroid drugs, beta-blockers and immunomodulators.
Discussion: The association of severe thrombocytopenia and Graves' Basedow disease is uncommon, its diagnosis during pregnancy is a clinical challenge, since it is necessary to rule out other gestational disorders. Therapeutic options in the first trimester of pregnancy are limited by the increased risk of teratogenicity.
Conclusions: The correct and timely diagnosis of this disease, allowed pharmacological decisions to be made in accordance with gestational age, thus achieving a pregnancy at term without complications.

Anti-NMDA receptor autoimmune encephalitis associated with systemic lupus erythematosus. Case report

Resumen
Introduction: Anti-NMDA autoimmune encephalitis is a severe neurological disorder characterized by autoantibodies targeting the N-methyl-D-aspartate receptor. Its diagnosis is challenging, and its coexistence with systemic lupus erythematosus is rare, posing significant clinical and therapeutic challenges.
Objective: To describe a case of anti-NMDA autoimmune encephalitis associated with SLE, highlighting diagnostic challenges and response to immunomodulatory treatment.
Case Presentation: Female patient, 26 years old, with a psychiatric history, admitted with altered level of consciousness and dystonic movements. Due to diagnostic suspicion, tests were performed and initially, anti-NMDA antibodies in cerebrospinal fluid were negative, which complicated the diagnosis. However, subsequent serum tests confirmed anti-NMDA encephalitis. At the same time, autoimmune activity consistent with neuro-lupus was identified according to the 2019 EULAR/ACR criteria, leading to treatment with methylprednisolone pulses and intravenous immunoglobulin, with significant improvement.
Discussion: The coexistence of anti-NMDA encephalitis and SLE is rare and may delay diagnosis when initial tests yield false-negative results. This case underscores the importance of diagnostic re-evaluations and a multidisciplinary approach to optimize treatment.

Resumen
Introduction: Research has identified certain key factors in human sexuality that can prevent health problems, such as HIV infection. Among these protective factors are self-efficacy to use condoms and sexual assertiveness. However, the existence of factors that promote risky sexual behaviors and that negatively impact emotional and mental well-being, in turn influencing general health, such as sexual depression, are also recognized.
Objective: determine the relationship between self-efficacy to use condoms, sexual depression and sexual assertiveness in migrants.
Material and methods: This is a correlational, cross-sectional study. The sample was made up of 249 people who migrate, who were selected from the population of migrants through non-probabilistic sampling, for convenience, in different social assistance centers for migrants. Three instruments were used to measure the variables. The Software Statistical Package for the Social Sciences version 20 was used to store and analyze the data. To know the characteristics of the participants, descriptive statistics and r-Spearman were used to determine the relationship between the variables.
Results: 85.40% were men, mostly from Mexico (84.70%). A relationship was found between self-efficacy for condom use with sexual depression and sexual assertiveness, however no relationship was found between self-efficacy for condom use with the dimensions of sexual assertiveness.
Discussion: The results obtained differ from some studies on sexual assertiveness due to various factors considering the study population.
Conclusion: self-efficacy to use condom increases sexual assertiveness. Sexual depression decreases sexual assertiveness.

Resumen
Introduction: More than 40 years after the emergence of HIV, it represents a global public health problem. Currently, condom use is the main means of protection, however the literature indicates that its use among young people is low.
Objective: To determine the relationship between perceived barriers to condom use and protected sexual behavior in young people.
Material and Methods:
This study was descriptive-correlational. The population consisted of young people from Chihuahua, Mexico. The sample was 129 and convenience sampling was used. Data collection was done online, and a sociodemographic and sexuality data sheet and two standardized subscales were used.
Results: The means of barriers to condom use was 13.5 (SD = 5.64), indicating that young people perceive few barriers to condom use. The means of protected sexual behavior was 39.4 (SD = 17.8), that is, young people have a low level of condom use. Barriers to condom use were identified as being negatively and significantly related to protected sexual behavior (rs = -0.255, p < 0.01).
Conclusion: The findings of the study confirmed the presence of individual barriers and the statistically significant relationships between these and condom use, knowing these limitations will allow them to be considered by health professionals in promoting the correct and consistent use of this means of protection.

Resumen
Introduction: Health literacy is key to the prevention and promotion of safe sexual behaviors to allow a decrease in the number of unplanned pregnancies and sexually transmitted infections among young people, situations that are considered public health issues. Objective: Describe the sexual behavior and sexual and reproductive health literacy among young people, compare sexual and reproductive health literacy by sex and sexual behavior, and examine the relationship between sexual and reproductive health literacy and the sexual behavior of young people.2Material and methods: The study design was descriptive, correlational and cross-sectional; the sample consisted of 622 young people from Mexico, selected at convenience. Data collection was performed through the Google Forms platform. Results: 83.6% of the participants had initiated a sexual life. The main sexual behaviors practiced by these young people include petting, watching pornography and masturbation; 52.9% reported that they changed condoms when they were going to have more than one type of sexual intercourse. The median sexual literacy in sexual and reproductive health was 48. Women had higher literacy scores than men as well as those who practiced petting and those who changed condoms when having more than one type of sexual intercourse. No relationship was found between sexual and reproductive health literacy and sexual behaviors.Conclusion: Young people show low sexual and reproductive literacy and engage in non-penetrative sexual behaviors. Although literacy is not related to sexual behavior, other factors are related such as age and sexual debut.

Resumen
Gender equity in science remains a challenge. Despite progress, barriers persist that limit women's access and recognition in research and scientific publishing. They represent only 28.4% of researchers and face difficulties in obtaining funding, promotions, and leadership roles.
From childhood, stereotypes discourage girls from pursuing STEM careers. In academia, women hold secondary roles in research and have less visibility in publications and editorial boards. The disproportion in awards and recognition highlights this inequity, as seen in the case of Esther Lederberg, whose contributions were overshadowed by her husband, who received the Nobel Prize.
In healthcare, although the number of women in academic programs has increased, their presence in leadership positions and specialties such as surgery remains low. This inequality affects both their professional development and the diversity of medical research.
Closing the gap requires strategies to encourage female participation in science, combat stereotypes, and ensure equitable conditions. Ensuring fair representation in editorial boards and funding is essential. Science will only reach its full potential when all voices are equally valued.

Resumen
Introduction: The development of social skills for a functional coexistence conditions a free and empathetic behavior, even in the face of difficult situations and gives a self-evaluative perception based on attitudinal, cognitive, affective and behavioral aspects, which enables a healthy self-esteem.
Objective: To analyze the psychometric properties of the Rosenberg self-esteem scale in university students in Ecuador.
Material and Methods: The general population sample consisted of 1146 young adults, students of the National University of Chimborazo, Ecuador, during the period from April 2023 to March 2024. The Rosenberg global self-esteem scale was used; confirmatory factor analysis, Kaiser Meyer Olkin correlation tests (KMO), and Varimax were applied using the Statistical Package for Social Sciences.
Results: The results indicated that the scale adjusted to a unidimensional model in the two groups studied, with confidence levels higher than the expected value (0.70). A confirmatory factor analysis with structural equations was established in case of bidirectional influences with factor loadings at 0.70. In the Kaiser-Meyer-Olkin test, a value of 0.842 and a p=0.00 was achieved, so the analysis was continued. Meanwhile, in the total variance explained with 68.386 %, the significance of the two types of self-esteem corresponded to 0.00. The Varimax index reported two principal components.
Conclusions: The questionnaire has psychometric properties that allow its application in the Ecuadorian population of young adults.

Resumen
Introduction: It is important to highlight the lack of information on specific interventions in the home setting for the adequate care of older adults with tracheostomy.Objective: The objective of this bibliographic review is to provide the most current evidence on nursing interventions provided to informal caregivers for the proper management of tracheostomy in the home setting.Material and Methods: A search was conducted in five electronic databases: PubMed, Scopus, Medline, SciELO, and Cuiden, with an additional search in Google Scholar. Articles published from 2014 to 2024 were selected. The PrismaCR analysis guidelines were used for the development of this systematic review.Results: Eleven original research articles were included, in which three main themes were identified: interventions for caregivers on the management of home tracheostomy, interventions for patients that promoted self-care on the management of home tracheostomy, and interventions for both patients and caregivers on the management of home tracheostomy.Conclusions: The reviewed interventions had positive impacts on increasing the level of knowledge and skills in patients and caregivers. They also improved the management of home tracheostomy, reducing levels of anxiety and burden in both caregivers and patients, as well as enhancing self-efficacy on the management of tracheostomy.

Resumen
Introduction: Determining overall cardiovascular risk is essential for developing prevention policies at the primary level of care.Objective: To design the global cardiovascular risk tables at the first level of care. Material and Methods: An applied, analytical, cross-sectional study was conducted with 585 people from three study groups at the primary level of care. A multivariate binary logistic regression analysis was carried out to develop the tables. They were compared with the tables of the World Health Organization and those of Framingham (Wilson) using Pearson's linear correlation coefficient and Cohen's kappa.Results: The 37.61% had a low risk level, 39.15% had a moderate risk level and 23.25% had a high-risk level with the primary care tables. The analysis of the equations showed a high positive correlation using Pearson's linear correlation coefficient. The agreement between the Framingham tables and the primary care tables was 81.03% and 71.62% with the World Health Organization tables. The area under the ROC curve was 0.90 for excellent discrimination.Conclusions: The global cardiovascular risk tables at the first level of care obtained good agreement, high correlation and excellent discrimination compared to the reference tables used as the gold standard.
